PRIORITY REGIONAL STRATEGY FOR IMPROVING THE ORGANIZATION OF MEDICAL CARE FOR PATIENTS WITH MALIGNANT NEOPLASMS OF THE DIGESTIVE SYSTEM

Summary:
Introduction. An increase in the number of cancer patients is registered worldwide every year. At the same time, the oncoepidemiological burden in malignant neoplasms (ZNO) of various localizations on different continents is very diverse and is associated with the degree of exposure to oncological risk factors, the unfavorable direction of demographic processes associated with aging and depopulation of the population, the quality of registration of cancer patients, as well as the level of resource base and the degree of perfection of the model of organization of medical care for cancer patients. In the Russian Federation, the results of complex multifactorial oncoepidemiological monitoring based on reliable data from population cancer registers are not sufficiently reflected in scientific research, and anti-cancer measures carried out at the regional level are often unidirectional rather than detailed. Purpose: formation of a regional panel of oncoepidemiological trends in malignant neoplasms of the Samara region for the scientific substantiation of the "Priority strategy for the development and improvement of medical care for patients with malignant neoplasms of the digestive system". Materials and methods. In the period 2012-2022 in the Samara region, the causes of the increase in the incidence of malignant neoplasms of the digestive system (C15-C26) were not only unfavorable demographic trends associated with aging and depopulation of the population, but also the additional impact of epidemiological risk factors. In addition to the increase in the incidence in older age groups, there was a significant increase in the number of cancer patients among the working-age population aged 40-49 years, and among women aged 30-39 years. Positive trends have been achieved only in terms of the quality of treatment and follow-up. Positive trends in diagnostic indicators have not been achieved. The organizational model of medical care for patients with digestive diseases has the 3rd degree of perfection (diagnosis is not effective + treatment is effective). A regional panel of oncoepidemiological trends in digestive diseases has been formed, which provides brief epidemiological characteristics and a list of priority (targeted) corrective measures to improve the quality of medical care. Conclusion. The completed research allowed us to form a strategy for the activities of the cancer service of the Samara region, developed within the framework of the national project "Fight against oncological diseases", to clarify, detail and supplement it with scientifically based priority measures for further improvement and development of medical care for patients with malignant neoplasms.
