RISK-BASED APPROACHES IN THE ORGANIZATION OF WORK ON ENDOSCOPIC DETECTION OF MALIGNANT NEOPLASMS OF THE DIGESTIVE TRACT IN THE SVERDLOVSK REGION

Summary:
Introduction. Risk-based approaches in healthcare are aimed at improving the effectiveness of diagnosis and treatment of various diseases, including cancer, at early, curable stages. Among the diagnostic studies of the gastrointestinal tract, endoscopic examinations are the most effective method for diagnosing precancerous and tumor pathology of the esophagus, stomach and colon. The purpose: To analyze statistical data on the detection of malignant neoplasms of the digestive tract in the Sverdlovsk region from 2019 to 2023, to compare with data on technical equipment, staffing and mortality dynamics. Materials and methods. A correlation analysis of the detection of cancer of the digestive tract at different stages, the level of technical equipment and staffing of endoscopic units of medical organizations in the Sverdlovsk region from 2019 to 2023 was carried out. The analysis was based on an assessment of medical-statistical data from the Information and Analytical System of the Endoscopy Commission of the Ministry of Health of the Russian Federation, data on the state of oncological care to the Russian population edited by A.D. Kaprin, data from annual reports on form No. 30 "Information about a medical organization" of the Medical Information and Analytical Center of the Ministry of Health of the Sverdlovsk Region, as well as data from the regional cancer registry. Results: An analysis of data on the technical equipment of endoscopic units in the Sverdlovsk region for 2019-23 showed that the number of gastroscopes meeting modern requirements increased by 150 units, which reduced the risks associated with equipping them with gastroscopes from 79.6% to 44.6%. The number of colonoscopes meeting modern requirements increased by 89 units, which reduced the risks associated with their equipment from 64% to 24.1%. The risk indicators for staffing were more than 75%. At the same time, in relation to the workload of full-time positions of endoscopists, these indicators were within acceptable and average values – less than 25% and up to 50%. Correlation analysis of mortality rates from malignant neoplasms of the digestive tract in the first year of life after diagnosis showed that the total risk decreases with an increase in the number of employed and staff rates of endoscopists in polyclinics (k=-0.92 and -0.86), with an increase in the number of diagnostic video/esophagogastroscopy and video/colonoscopy (k=-0.68/ 0.79 and -0.72/0.88). Discussions: A correlation analysis of the total mortality risk from malignant neoplasms of the digestive tract in the first year after diagnosis showed a tendency to decrease with the introduction of video endoscopic equipment and improved staffing of endoscopic clinics. Conclusion: An analysis of the technical equipment and staffing of endoscopic units in the Sverdlovsk region confirmed the effectiveness of measures to organize the work of endoscopic units aimed at reducing mortality from malignant neoplasms of the digestive tract.
