ORGANIZATIONAL MEASURES TO IMPROVE EMPIRICAL ANTIBACTERIAL THERAPY IN PATIENTS WITH INFECTIONS ASSOCIATED WITH PREVIOUS HOSPITALIZATION

Summary:
Introduction. The set of measures required for medical organizations (MO) to improve the quality of use of antibacterial drugs in the Russian Federation is designated as the «Strategy for Control of Antimicrobial Therapy» (SCAT). Today, given the rapid spread of microorganisms with multiple drug resistance, the increase in the volume and types of medical care, the intensification of surgical treatment methods, additional organizational measures are required to improve the effectiveness of SCAT.
Purpose. Description of a case study on the development and implementation of a set of organizational measures aimed at increasing the effectiveness of empirical antibacterial therapy (ABT) in patients with surgical site infections associated with previous hospitalization (SSI).onal organizational measures are required to improve the effectiveness of SCAT.
Materials and methods. The study examined cases of patients with SSI (n=100) and documentation regulating the activities of the medical organization on the prevention and treatment of SSI. Problems in the medical organization were identified using lean manufacturing tools. Then, a set of organizational measures was developed and implemented in the medical organization to improve empirical ABT in patients with SSI under the conditions of an organizational experiment.
Results and discussion. Low detection rate of SSI (as a nosocomial infection) by surgeons at the stage of patient admission to the emergency department was revealed (out of 100 cases of SSI, 23 cases were transferred to an epidemiologist for further observation). In 35.0% of cases, a change in antibiotic therapy was observed. Problems associated with the ethological decoding of SSI and the logistics of obtaining the results of bacteriological testing were identified - in 19.0% of cases the result was lost and a duplicate was required. At the stage of discharging a patient with SSI, it was found that in 43.0% of cases, the discharge summaries did not contain a specific list of names of antibiotics prescribed to the patient. Taking into account the identified problems, a set of organizational measures was formed in four main blocks: detection, recording and registration of SSI (detection algorithm, employee training program, control system); appointment of empirical antibacterial therapy (antibacterial therapy protocol for SSI in the regional medical institution, control system); microbiological monitoring (entering pathogens into the medical information system, recording pathogens in the microbiological monitoring system); ensuring continuity in the provision of medical care to patients with SSI (scheme of functional interaction of specialists, patient card with surgical infection).
