LABORATORY PROFILE OF POTENTIAL PREDICTORS OF TARGETED BIOLOGIC ASTHMA THERAPY IN PERSONS OVER 60 YEARS OF AGE

Summary:
Introduction.. In 2023, on the basis of St. Petersburg State Budgetary Healthcare Institution "City Multidisciplinary Hospital No. 2," an Expert Center for the profile "Pulmonology" (PC) was opened, one of the tasks of which is the use of high-tech treatment methods, such as targeted genetic engineering biological therapy (GEBT). A comprehensive evaluation of the efficacy and safety of GEBT treatment remains an urgent task. Of particular interest is the strategy for the treatment of elderly and senile patients who are characterized by a pro-inflammatory background (inflammaging), comorbidity and polypharmacy.
The purpose of the study: study the features of laboratory support depending on the level of control of the course of bronchial asthma during the use of GEBT (using the example of dupilumab) in people over 60 years of age.
Materials and methods. The study involved 86 patients diagnosed with J45.1 "Non-allergic asthma" who were treated with dupilumab in the PC in 2024. Asthma control at 3 months with GEBT was assessed using the Asthma Control Test (ACT) questionnaire.
Results. In the group of patients with controlled course of bronchial asthma, dupilumab treatment revealed lower baseline albumin-globulin index (AG) values, which was also typical for people over 60 years of age. In addition, in elderly patients, compared with the general cohort, significantly higher indicators of this index were established, which may indicate the initial pro-inflammatory background in the older age group.
Discussion. A low AG value in patients with severe asthma compared to the reference indicates an important role of inflammatory processes in the pathogenesis of this disease. Statistically significant differences in hypertension depending on the level of disease control against the background of GEBT indicate the presence of individual characteristics, including those related to age, which must be taken into account when prescribing targeted therapy.
Conclusion. Thus, a potential predictor of the controllability of the course of bronchial asthma with dupilumab is AG: at first, a lower level of this index correlated with high asthma control in all age groups.
