Summary:
Introduction. More and more cancer patients are registered worldwide every year. The epidemiological situation in malignant neoplasms (MN) of different localizations in different continents is diverse and is associated with the impact of risk factors for oncological pathology, the unfavorable nature of the medical and demographic processes of aging and depopulation of the population, the organization of registration of oncological patients, the possibilities of resource provision, as well as the accepted organizational models of medical care for oncological patients. In Russia, complex multifactorial epidemiological monitoring based on data from population cancer registers is still not actively conducted. As a result, anti-cancer measures, including those carried out within the framework of national projects at the regional level, are often not specific, but general.
Purpose: oncoepidemiological characteristics of trends in the development of female genital and breast cancer in the Samara region for the scientific justification of the strategy for the development of medical care for women with malignant neoplasms.
Materials and methods. The study was conducted on the basis of the Samara Cancer Registry. The study program included a sample of 31911 women suffering from genital cancer of various localizations (14637 patients) and breast cancer (17274 patients), registered in the period 2012-2022. Trends in morbidity and quality of medical care were studied. The work uses traditional methods of statistical analysis, as well as the author's methods. An oncoepidemiological characteristic has been obtained and a list of priority corrective measures has been scientifically substantiated to improve the level of medical care for women with malignant neoplasms of the female genital organs and breast.
Results and discussion. During 2012-2022, there was an increase in the incidence of female genital and breast cancer in the Samara region, which was mainly associated with increased exposure to epidemiological risk factors. A comparative analysis of trends in standardized morbidity and mortality rates in the most significant localities in women (breast, endometrial, cervical, ovarian, vulvar, and vaginal cancers) showed a positive relationship between them - "an increase in morbidity and a decrease in mortality," which indicated a satisfactory quality of medical care. In general, the proportion of 1-2 st. of the process (71.9±1.7%) increased by 6.4%; 1-year mortality (15.7±1.1%) decreased by 24.1%; The proportion of patients registered for more than 5 years (65.9±0.4%) increased by 1.9%. In terms of 1-year (84.4±1.2%), 3-year (70.4±2.4%) and 5-year (64.7±2.3%) relative survival rates, positive growth trends were noted – by 5.9%, 13.3% and 11.0%, respectively. In terms of neglect (15.9±1.0%), positive trends could not be achieved (an increase of 6.9%), despite a significant increase in the number of actively identified patients by 1.2 times. In breast cancer, negative trends were registered not only in the indicator of neglect, but also in the indicator of early diagnosis.
Conclusion. Thus, the varying degrees of perfection of the medical care system provided to women with cancer are shown. The epidemiological characteristics of trends for the most significant cancer localities in women are presented, which make it possible to improve the strategy of the oncological service of the Samara region, developed within the framework of the national project "Fight against oncological diseases".
