MEDICAL AND SOCIOLOGICAL RESEARCH OF DOCTORS ON THE USE OF INFORMATION TECHNOLOGY IN IMPROVING THE ORGANIZATION OF MEDICAL CARE

Summary:
Introduction. Modern information solutions create the basis for increased expectations from both medical staff and patients. As a result, the issues of the development of information (digital) technologies with their inclusion in the models of improving the organization of medical care in various conditions are becoming very relevant. With the development of information technology (IT) in the healthcare sector, when improving the organization of medical care, an important role belongs to doctors who are directly involved in the implementation of modern digital solutions, the study of whose opinions is of considerable interest.
Purpose: to conduct a medical and sociological study of doctors on the use of information technology in improving the organization of medical care in a city hospital.
Materials and methods. The Novokuibyshevsk Central City Hospital (NCGB), a state budgetary healthcare institution in the Samara region, providing medical care to the population of Novokuibyshevsk (average annual population of 100 thousand inhabitants), was chosen as the research base. Novokuibyshevskaya Central Clinical Hospital is a united hospital that provides medical care in outpatient settings (the capacity of the polyclinic is 1,150 visits per shift) and inpatient settings (a 24-hour hospital with 430 beds). In the course of the conducted medical and sociological research, 206 NCGB doctors were surveyed on the issues of exploring the possibilities of using information technologies to improve the organization of medical care according to a specially developed questionnaire. The research is based on the use of analytical, sociological and statistical methods.
Results and discussion. All the doctors noted that they are aware of certain information technologies that are used or could be used in the healthcare system. On average, respondents indicated 2.2 information technologies in healthcare known to them per respondent. According to the overwhelming majority of NCGB doctors surveyed (85.9%), information technology is being used to a sufficient extent in the city hospital. The rest of the respondents (14.1%) noted insufficient use of IT. Of those doctors who note the insufficient use of information technology at the present time, the main reasons for this are a shortage of qualified personnel (96.6%), insufficient material and technical support (72.4%), low level of training of medical personnel (44.8%) and a small number of information products themselves (41.4%). According to the survey, 58.3% of NCGB doctors are completely satisfied with the work of the unified medical information and analytical system of the Samara region (EMIAS), another 29.1% of respondents are more satisfied with the work of EMIAS than dissatisfied. However, 12.6% of doctors are more likely to be dissatisfied with how this system works. The main reasons for dissatisfaction with the work of the EMIAS are the insufficient productivity of the information system, accompanied by "low speed of work" (65.4% of responses), insufficient coverage of all services of the city hospital ("insufficient functionality") (30.8% of responses), the presence of interruptions in the work of the IIA ("unstable work") (15.4% of responses).
Conclusion. Doctors see the main directions of using information technology in the organization of outpatient medical care in creating patient registers (76.2% of responses), making medical decisions (46.1%), improving preventive measures (42.7%), improving preferential drug provision (40.8%), etc. Among the main areas of information technology use in the organization of medical care in inpatient settings, doctors note medical decision-making (75.7% of responses), hospital admission management (73.3%), improved continuity with outpatient care (71.8%), and improved drug provision in the hospital (51.5%). Thus, sociological approaches to the organization of medical care for patients allow us to obtain additional data to improve its provision and information support.
