MATERNITY HOUSES INDICATORS OF THE KAMCHATKA TERRITORY IN 2022 - 2024: A CROSS-SECTIONAL STUDY

Summary:
Introduction. Building a modern maternity care system based on up-to-date scientific data and modeling of major demographic, medical, organizational and managerial trends plays a key role in ensuring maternal and child health, reducing maternal and perinatal mortality, and improving the quality of medical care.
The purpose of the study is to study trends in the organization of maternity services in the Kamchatka Territory (KТ) based on data for 2022-2024.
Materials and methods. A cross–sectional study was conducted using a continuous sample of patients based on the reports of the maternity hospitals of KТ in accordance with the completed forms 32 and the insert of form 32 for 2022-2024.
Results. In the maternity hospitals, women had pre–existing arterial hypertension, which complicated the course of pregnancy, childbirth, and the postpartum period, in 2022 – 168 (5.7%), in 2023 – 293 (10.1%), in 2024 - 190 (7%) cases. Severe and moderate preeclampsia was detected in 2022 in 80 women (2.7%), in 2023 in 146 women (5%), in 2024 in 136 women (5%). Diabetes mellitus in the maternity hospital in 2022 was detected in 164 women (5.6%), in 2023 – in 311 (10.7%), in 2024 – in 231 women (8.6%).
Discussion. Despite the decrease in the number of births over the past 3 years (2022-2023-2024) (2924-2901-2699), the availability of hospital beds remains the same (0,0380-0,0378-0,0378 ). Since 2022, there has been a threefold decrease in the number of women using intrauterine devices (10286-9816-3387) and using hormonal contraceptives (10195-11043-5624) by 1.8 times. The incidence of urinary tract infections, hypertension, and anemia is increasing (5%-6,9%-8,8%, 2,1%-4,1%-6,7%, 18,1%-24,1%-29,5%).
Conclusions. Despite a decrease in the number of women using contraception in the form of intrauterine devices by 3 times and hormonal contraceptives by 1.8 times, there is a decrease in the number of births and children born. Against the background of an increase in the number of induced labor (3.4 times), there is a decrease in the number of births over 42 weeks (4 times). Despite a 1.4-fold increase in the number of births resulting from IVF, which often involves the insertion of multiple embryos, the number of multiple pregnancies has not increased. There is a 1.6-fold decrease in the frequency of participation in home delivery by medical and nursing staff, which increases the risk and likelihood of adverse outcomes. There is an increase in the number of women in labor with hypertension (1.2 times), preeclampsia (almost 2 times), diabetes mellitus (1.5 times), urinary tract infections (1.8 times), venous pathologies (3.2 times), anemia (1.6 times).
