Summary:
Introduction. With the demographic aging of the population, there is a need to assess the functional status of older adults as a key component of health-related quality of life. The ICOPE (Integrated Care for Older People) questionnaire, developed by the World Health Organization, is currently the methodological basis for screening the functional status of older adults. Analyzing this instrument, we need to consider the possibility of modifying it to take into account regional specificities, using the example of the Republic of Ingushetia, characterized by a high proportion of the rural population. Identifying the prevalence of geriatric syndromes and risk factors for their development requires the development of modified screening tools adapted to local conditions.
Objective. To consider the possibilities of expanding and modifying the ICOPE questionnaire for screening the functional capacity of elderly patients, taking into account the regional characteristics of the Republic of Ingushetia.
Materials and methods. The study was conducted in two sequential stages. The first stage analyzed the validation of the basic version of the ICOPE screening system in primary healthcare facilities in the Voronezh Region. The second stage included a pilot screening study using the ICOPE questionnaire, supplemented with consideration of the local living conditions of elderly people in the Republic of Ingushetia. The study included 37 elderly and very old respondents permanently residing in the Republic of Ingushetia. The average age was 74.2±8.6 years. Physicians from outpatient clinics administered questionnaires to patients using the questionnaire for studying the functionality of elderly people.
Results and discussion. A pilot study using an expanded questionnaire assessed the respondents' functional status. A high prevalence of pain syndrome (95.6%), limited mobility (48.6%) of respondents, 37.7% reported falls were revealed. Features of the nutritional status were revealed - 36.4% noted a weight loss of about 3 kg over the past three months, 43.6% of respondents indicated nutritional problems. It is noteworthy that when assessing dental health, 86.4% of elderly people have limited access to dental care. Screening assessment of the cognitive sphere showed that 68.5% of respondents noted memory impairment. Vision difficulties were noted by 47.2% of respondents, hearing loss - 39.6% of respondents. A high prevalence of psycho-emotional problems was revealed: a feeling of depression was noted by 71.2% of respondents, a decrease in interest in usual activities - 58.9%. In the surveyed group, 16.3% of respondents noted instances of discrimination and violence.
Conclusion. The results of the pilot study confirmed the feasibility of expanding and modifying the ICOPE questionnaire for screening the functional capacity of older adults living in the Republic of Ingushetia. The identified high prevalence of impairments in the domains of intrinsic capacity justifies the inclusion of additional questions reflecting regional and socio-environmental factors. The modified ICOPE questionnaire provides a methodological basis for developing programs aimed at improving the functionality, vitality, and quality of life of older adults in specific socio-environmental conditions.
