COMORBIDITY OF PSYCHOEMOTIONAL PROBLEMS IN ADOLESCENTS: EPIDEMIOLOGICAL DATA AND ORGANIZATIONAL IMPLICATIONS

Summary:
Introduction. The increasing prevalence of psychoemotional disorders among adolescents and the high proportion of comorbid conditions place an additional burden on the system of prevention and psychological/psychotherapeutic care within the educational environment. The simultaneous presence of anxiety, depressive, suicidal/auto-aggressive manifestations, and а tendency toward risky behavior is associated with а more severe course of the disorder, adverse social consequences, and the need for interdepartmental referral pathways. Objective. To analyze the prevalence and pattern of co-occurring symptoms of psychoemotional disorders in adolescents and to assess the organizational implications of the identified combinations for planning preventive measures. Materials and methods. А cross-sectional school-based survey was conducted in Kazan (Russia). This publication reports an analysis of а subsample (n=975) drawn from а larger school screening dataset (N=1477) and restricted to respondents aged 14–17 years with complete key fields. А self-report questionnaire with 60 dichotomous items grouped into six subscales was used; threshold values were applied to flag а subscale-positive symptom profile (А–Г: ≥6 points; Д–Е: ≥3 points). We calculated the prevalence of individual subscales, the number of concurrent subscales flagged, and the most frequent combinations. Results. А significant proportion of adolescents showed signs of emotional distress: 30% had at least one sign, and 11.9% had three or more signs. Combinations of two or three signs were the most common, while groups with four or five signs were less frequent but characterized by high organizational significance due to the need for multi-level support. Typical combination profiles were identified, allowing for the selection of high-risk groups for targeted interventions. Discussion. The obtained data confirm the feasibility of regular school-based screening with а focus on multi-symptom conditions, the development of interdepartmental pathways (school — family — medical organization), the standardization of referral algorithms to specialists, and the implementation of remote psychoeducational tools while adhering to the principles of confidentiality and evidencд-бased prevention. Conclusions. Analyzing the pattern of co-occurring symptoms in adolescents can be used to improve early detection programs, plan staffing and organizational solutions, and increase the accessibility of psychotherapeutic care for schoolchildren.
