DYNAMIC RANKING OF SUBJECTS OF THE VOLGA FEDERAL DISTRICT ACCORDING TO THE AVAILABILITY OF SPECIALIST PHYSICIANS PROVIDING MEDICAL CARE FOR DISEASES OF THE CIRCULATORY SYSTEM

Summary:
Introduction. In order to achieve the targets and additional indicators of the national projects «Healthcare» and «Long and active life», it is necessary to predict the needs of the regions for specialist physicians providing specialized medical care to patients with circulatory diseases. In the context of the problem under consideration, the dynamic ranking of the subjects of the Volga Federal District according to the provision of specialist physicians to the population meets several tasks, including identifying the interregional differentiation of indicators, determining the potential of a targeted focus strategy for various types of regions and the vector of regional development to ensure better and more effective medical care.
Purpose of the investigation. To rank the subjects of the Volga Federal District according to the availability of specialist physicians providing medical care for diseases of the circulatory system, to evaluate the results of the dynamics of the ranking distribution.
Materials and methods. The study of the dynamics of population availability of specialist physicians was conducted taking into account the general principles of dynamic series analysis, with the calculation of growth coefficients and basic growth rates. For the final ranking, the method of direct ordinal ranking was applied by two reference points with segmentation of territories in descending order.
Results and discussion. An upward change in the ranking of the population's provision of cardiologists by more than 5 points was registered only in 2 regions: in the Kirov Region (+7 points) and in the Republic of Tatarstan (+6 points). The transformation of the population's provision of neurologists by more than 5 points is typical for 4 regions of the Volga Federal District, while 2 regions have improved their positions. Significant positive changes in the ranking positions of the population with rheumatologists are registered only in the Republic of Tatarstan (+5,5 points). An increase in the population's provision with rheumatologists is observed in 2 subjects of the Volga Federal District: in the Perm Region (+3,5 points) and the Penza Region (+3 points). The composition of the leaders in terms of population availability of cardiovascular surgeons is stable. The regions with the lowest values of the index of the sum of ranks include 2 subjects of the Volga Federal District: the Udmurt Republic (Isum=0; sumR=29,97) and the Republic of Mordovia (Isum=0,04; sumR=32,76). In the total order, 4 regions (Isum range from 0,22 to 0,37) occupy intermediate ranking positions: Kirov Region, Orenburg Region, Republic of Tatarstan and Penza Region. The least prosperous regions according to Isum include 8 subjects of the Volga Federal District with values ranging from 0,43 to 1.
Conclusion. The study established: a change in the positions of the leading regions in terms of provision of cardiologists; a redistribution of the regions of the Volga Federal District in terms of provision of neurologists in 4 subjects; an increase in the positions of the Penza region in terms of provision of rheumatologists; stability of the rank distribution of the regions of the Volga Federal District in terms of provision of cardiovascular surgeons. Final ranking distribution of subjects of the Volga Federal District by availability of specialist physicians: 2 regions - 1-2 places, 4 regions - 3-6 places, 8 regions - last positions.
