ANALYSIS OF MEDICAL CARE FOR PATIENTS WITH FRACTURES OF THE PROXIMAL FEMUR

Summary:
Fractures of the proximal femur occur mainly in the elderly. Of all the common types of isolated skeletal injury in everyday life, OCD poses the greatest danger to the life, activity, and self-care abilities of senior citizens. The dynamics of the epidemic situation of femoral fractures makes it possible to predict an increase in the number of these injuries, especially pronounced in people over working age.
Purpose: analysis of care for patients with fractures of the proximal femur in three federal districts of the Russian Federation for 2023-2024.
Materials and methods: the reports provided using the automated information system for collecting reports (AIS SO) by the heads of specialized structural divisions providing inpatient and outpatient care are analyzed. The reports contain information on the number of referrals for fractures of the proximal femur with the proportion of hospital referrals, the number of hospitalizations, surgical activity and the proportion of endoprosthetics. The data obtained from 220 inpatient units in 2023 and 206 in 2024, as well as from 210 outpatient units in 2023, were studied. and 189 for 2024. The share of medical organizations that provided reports was 58.7% for 2023 and 49.9% for 2024.
Results: the proportion of patients with fractures of the proximal femur referred for hospitalization was 70% in 2023 and 73.8% in 2024. Surgical activity in hospitals averaged 75.1% in 2023 and 77.5% in 2024. The share of endoprosthetics out of the total number of operations averaged 31.6% in 2023 and 36.7% in 2024. Significant variability was noted in the regions.
Conclusions: with the limitations of AIS at this stage, it is possible to obtain updated information on the state of care for fractures of the proximal femur at the level of the federal districts of the Russian Federation. Based on the analysis of the information received, problematic aspects of the state of trauma and orthopedic services in certain regions can be identified, and decisions aimed at improving the quality of medical care have been developed and adopted. In the future, with an increase in the degree of AIS occupancy, information about the epidemiology of the injury in question can be obtained, the analysis of which will reveal its regional characteristics.
