Abstract. The study of medical and social expertise of the elderly is relevant due to the increasing proportion of the elderly population in the population. Objective: To analyze the structure of disability in the elderly population (over 60 years old) of the Arkhangelsk region due to pathology of the musculoskeletal system and rehabilitation measures in 2019-2023. Materials and methods. The study was based on reports on medical and social expertise conducted by the Bureau of Medical and Social Expertise of the Arkhangelsk Region. Analytical and statistical methods were used, calculations of extensive and intensive indicators, absolute values of primary and total disability. Non-personalized data of people with disabilities were used. Results: From 2019 to 2023, there was a tendency for the number of people with disabilities due to musculoskeletal system (MSD) pathology among the elderly to decrease (2019-2022) and then increase (2023). Intensive indicator of general disability according to the MSD among the elderly and senile population. When analyzing the structure of primary disability by group, people with the third disability group take the lead, with 584 people (69.5%). Women take the lead in all three disability groups, and the rates among women are three or more times higher than those among men in each group. The most common reason for disability were coxarthrosis, gonarthrosis, and polyarthrosis. The number of the first group is approximately equal in different age groups; the number of the second and third group is approximately five times higher in the 60-74 age group compared to the 75-89 age group. When analyzing the proportion by place of residence, a higher number of newly recognized disabled people is observed among the urban population. Conclusion: To improve the effectiveness of diagnostics and reduce primary disability, it is advisable to implement unified examination standards, strengthen preventive work, and expand the availability of specialized care, including modern methods of visual
