Summary:
Introduction. Reducing mortality from cardiovascular diseases (CVDs) is a key factor for increasing the life expectancy of citizens of Russia. Assessment of its dynamics is necessary to identify at-risk population groups and to improve the provision of specialized medical care to patients.
Purpose of the study was to identify the features of the nosological structure, intra- and intergenerational dynamics of mortality from CVDs in the population of the Komi Republic (North-West Russia).
Materials and methods: A retrospective epidemiological study was conducted using data from federal statistical observation forms. Statistical analysis involved the calculation of crude and standardized mortality rates from CVDs. The Age-Period-Cohort (APC) analysis was applied to determine the parameters of intra- and intergenerational dynamics of the crude mortality rates from CVDs.
Results and discussion. CVDs remain the leading cause of death in the population of the Komi Republic. In 2011–2022, the share of cardiovascular diseases (CVD) in the nosological structure of causes of death among men ranged from 34.8% to 43.3%; among women, from 41.2% to 53.0%. The CVD mortality rate in the Komi Republic increased by 19.6% between 2017 and 2022, and after 2021, it has exceeded that of the country's population as a whole. In 2011–2022, CVD mortality among men, as well as among residents of rural settlements of the Komi Republic, was higher in comparison with mortality from the same group of conditions among women and residents of urban settlements, respectively. The pronounced positive "birth cohort" effect in CVD mortality dynamics confirms an increasing risk of fatal outcome with advancing age in both men and women; the absence, or the negligible scale, of an "periodic" effect indirectly suggests that the non-communicable disease prevention programs implemented in the region and/or the accessibility of specialized medical care have not been sufficiently effective.
Conclusion. The study results can be used in developing and adjusting regional programs for the prevention of CVDs and demographic development, as well as in improving the organization of healthcare delivery to the population.
