Summary:
Resume. Introduction. Bone loss in the facial skull and jaws is a systemic and local process that can form a phenotype of premature facial aging, when external age-related changes are more pronounced than expected by chronological age. The development of this phenotype is due to three main mechanisms: alveolar resorption in edentia and periodontitis; decreased bone mineral density of the facial skeleton due to endocrine-metabolic disorders (estrogen/testosterone deficiency, vitamin D, hyperparathyroidism, thyroid dysfunction); drug-induced bone loss due to polypharmacy (glucocorticoids, proton pump inhibitors). These changes have not only aesthetic but also functional significance: impaired chewing, malnutrition, deterioration of diction, and social isolation. Objective. To study the gerontological manifestations of the phenotype of patients with bone loss of the facial skull and jaws, leading to premature aging. Materials and methods. The study included 76 patients (38 men, 38 women) aged 50 to 79 years. The average age was 64.6 ± 7.5 years. Distribution into two age groups: 50-64 years - 38 people (50%), 65-79 years - 38 people (50%). Inclusion criteria: age 60 years and older, the presence of external morphological signs of supporting bone changes. Anamnesis, including drug and dental status, was studied in all patients. Using laboratory and instrumental diagnostic methods, mineral metabolism and mineral density of bone tissue were assessed. Results and discussion. The study revealed significant gender differences in the perception of age-related aesthetic changes in the face, more pronounced in women (89.5%). 68.4% of patients were completely or partially edentulous, with the average duration of tooth loss being 7.2±5.8 years. Periodontitis was detected in 76.3% of patients, and 23.7% of edentulous patients lacked dental prosthetics. These problems accelerate alveolar resorption, causing irreversible jaw bone atrophy, resulting in a shortened lower third of the face, a reduced bite height, and loss of full chewing function. An assessment of mineral metabolism and bone resorption parameters in different age groups revealed a statistically significant decrease in bone mineral density in patients aged 65-79 years, with hypocalcemia and hypophosphatemia occurring in 13.2% of patients compared to 2.6% in the 50-64 age group (p
