Summary:
Introduction. Recently, in the Russian Federation, there has been a trend towards an increase in the incidence of glaucoma. This phenomenon is due to a combination of factors, including both objective demographic changes and improvements in methodological approaches to diagnosis. The rise in glaucoma incidence across various demographic and clinical groups affects not only statistical indicators but also the quality of life of patients. The chronic and progressive course of glaucoma is one of the main causes of irreversible blindness, which leads to disability among people in the older age group.
Purpose: to study the medical and social aspects of the incidence of glaucoma in patients attending outpatient clinics at the regional clinical ophthalmological hospital.
Materials and methods. The study was conducted at the glaucoma ophthalmological consultative and diagnostic department of the Samara Regional Clinical Ophthalmological Hospital named after T.I. Eroshevsky. This department is part of the outpatient clinic unit of the regional hospital, which provides specialized ophthalmological care in outpatient settings. Medical and statistical indicators were assessed using data from the Unified Medical Information and Analytical System and the federal statistical reporting form No. 12 “Information on the number of diseases registered in patients residing in the service area of a medical organization” for the period 2012-2024. Statistical and analytical research methods were used.
Results and discussion. Between 2012 and 2024, the advisory clinic of the regional ophthalmological hospital recorded 11,348 visits out of 16,014 appointments. Among all visits, 1,580 cases concerned glaucoma, which accounts for 13.9% of the total number of requests for specialized ophthalmological care. The majority of patients in the glaucoma department (72.5%) lived in the city of Samara; women accounted for 61.5% of the total number of patients; a predominance of patients in the age category from 71 to 80 years was identified, making up 37.6% of the total. Of the 1,580 patients examined, 461 (29.2%) had been diagnosed with diabetes. The majority of patients (65.3%) already had an established diagnosis of glaucoma or were at the preliminary diagnostic stage. In the structure of glaucoma pathology, primary open angle glaucoma (POAG) dominated, accounting for 41.7% of the total number of cases. Primary closed angle glaucoma (CCAG) was diagnosed in 16.5% of patients. The category of patients with suspected glaucoma included 23.6% of those who sought medical attention. In 15.7% of cases, unspecified glaucoma was observed, which required additional diagnostic measures and the development of an individual treatment strategy. Secondary glaucoma (SG), which occurs against the background of other ophthalmological diseases, was significantly less common, accounting for only 2.5% of the total number of observed cases.
Conclusions. Every seventh case of referrals (13.9%) to the outpatient department of the regional ophthalmological hospital is associated with glaucoma. Among patients with glaucoma, as well as those with a combination of glaucoma and diabetes mellitus, individuals of pre retirement and retirement age predominated throughout the study period, most often women living in the regional center, which corresponds to the existing medical and statistical trends characteristic of the Russian Federation as a whole. The data obtained can be used in the future to predict glaucoma incidence based on patient registration data, to develop treatment and diagnostic strategies and follow up care plans, and to formulate economic cost plans for providing high quality ophthalmological care to the adult population.
