Summary:
Cosmetology is one of the most dynamically developing areas of medicine, in which patient satisfaction plays a key role due to the voluntary nature of seeking care and the highly subjective nature of aesthetic outcome assessment. Objective. To assess satisfaction with the quality and safety of medical care of patients and medical staff of a cosmetology clinic as an element of the internal control system using an integrated approach, including stratification by risk clusters, correlation analysis and data triangulation. Materials and methods. An anonymous survey was conducted among 378 patients and 39 employees of a healthcare organization providing outpatient cosmetology care. Patients were stratified into four risk clusters depending on the risk associated with the procedures, correlation analysis and one-way analysis of variance with post-hoc comparison were performed. Survey data were triangulated with the results of an internal audit of medical documentation. Results. Patients highly rated the overall comfort in the clinic (4.5 out of 5 points) and the aesthetic result (4.2 points), but noted significant deficiencies in post-procedural support (3.5 points) and comprehensiveness of information (3.8 points). When stratified by risk clusters, a statistically significant decrease in satisfaction was found with increasing invasiveness of the procedures: in the high-risk cluster, comprehensiveness of information was rated at 3.0 points, post-procedural attention at 2.8 points, and compliance with expectations at 3.2 points. The staff critically assessed the internal control system: internal audit — 1.8 points, safety culture — 2.2 points, standardization — 2.7 points. Correlation analysis revealed strong relationships between comprehensiveness of information and compliance with expectations, as well as between a sense of safety and willingness to recommend the clinic. A qualitative analysis of open-ended staff responses revealed the root causes of these defects: time constraints in eighty percent of respondents, inconvenient documentation in seventy percent, and communication barriers in fifty percent. Discussion. The obtained results are consistent with data from Russian and foreign studies and complement them. The critical role of information provision in managing patient expectations and building loyalty was established. The phenomenon of the "illusion of safety" was identified, where patients feel protected based on external signs, while staff assess the risk management system as extremely ineffective. The need for a differentiated approach to quality management for different procedure clusters was confirmed. Conclusion. The internal quality control system in the studied clinic is fragmented: despite a high level of equipment and comfort, key processes — standardization, information provision, post-procedural management, and error analysis — are implemented to a limited extent. The identified problem areas require the development of a risk-based model for internal quality control and medical safety with differentiated standards for different procedure clusters.
