THE RESULTS OF THE PROPHYLACTIC MEDICAL EXAMINATION 2013-17 GG.: THE DISTRIBUTION IN HEALTH GROUPS

Summary:
Introduction. The task of preserving the health of the population is currently considered today as one of the primary tasks of the state, which is reflected in the may presidential decrees of 2018, in the program "development of health care" and other governing documents of the modern Russian Federation. The preventive direction is intended to become a key in the activities of medical organizations, and medical examination of the population is one of its main tools.
The purpose of this study was to analyze the indicators of coverage of the adult population of the Russian Federation with medical examination for the period 2013-2017 and the distribution of the surveyed citizens by health groups.
Material and methods. The method of descriptive statistics is used in the analysis of indicators. Calculation of indicators of coverage of the population of DVN was made on the basis of the adult population of the Russian Federation: for the cycle of 2013-2015-on January 1, 2016, for the cycle of 2016-2017 - on January 1, 2018. The calculation of indicators related to the distribution by health groups was based on the number of the surveyed population.To analyze the distribution of persons who passed the 1st stage of DVN, by sex and age, the corresponding shares of the total population were calculated: the ratio of the number of persons who passed the 1st stage of medical examination to the total population of the corresponding age group (respectively, Russia or FO). The proportion of the population classified as 1, 2 or 3 (3A, 3b) health groups was defined as the ratio of the relevant health group to the total population of the past DVN.
Results and discussion. In General, in Russia in the first cycle (2013-2015) passed DVN 64.3 million citizens (53% of the adult population); in the second – "unfinished" – cycle 2016-2017 – 43.6 million people (41% of the total adult population). Thus, the total number of surveyed for the period amounted to more than 107 million people. The highest rate of DVN coverage is in the 21-36 age group. In all age groups, the proportion of women surveyed is higher than that of men. For the period 2013-2015 1 health group was established 34%, 2 health group - 21%, 3 group - 42% of the total number of citizens who have passed DVN. According to the results of 2016-2017, the citizens who underwent medical examination were distributed as follows: 1 group - 30%, 2 group – 18%, 3A group – 41% and 3b group – 11% (i.e., total – 52%) of the total number of persons surveyed under the DVN program. It can be noted that the distribution of the surveyed population by health groups was characterized by a decrease in the second cycle compared with the first proportion of the population classified as 1 and 2 health groups, and an increase in the proportion of the population classified as 3 health groups.
Summary. During the period 2013-2015, the survey within the medical examination of certain groups of the adult population covered 53% of the adult population; for the second, "unfinished" three – year cycle of medical examination (2016-2017) - 41% of the total adult population. The analysis of statistical data showed that along with extensive indicators of implementation of the plan of performance of medical examination it is necessary to study intensive indicators as they reflect a real picture of coverage of the population by medical examination. Especially important differences were obtained in the analysis of indicators in the context of sex and age groups. This points to the need for more careful planning of medical examination of the adult population, including the need to take into account the gender and age characteristics of the population of specific territories. In all age groups, the proportion of women in the total population surveyed is higher than that of men. At the same time, women in all three age groups considered in the framework of medical examination showed a small excess of the proportion of persons classified as 2 and 3 groups of health, compared with the male population. This, in turn, points to the likely gender differences in the real need for health care, including preventive care, and requires further study. The distribution of the surveyed population by health groups was characterized by a decrease in the cycle of 2016-2017 compared with the results of 2013-2015. the proportion of the population assigned to health groups 1 and 2 and the increase in the proportion of the population assigned to health group 3. The revealed differences in volumes and results of medical examination of certain groups of the adult population between Federal districts and subjects of the Russian Federation cause expediency of the detailed analysis of quality of planning and carrying out medical examination, and also quality of the account of its results for the purpose of further improvement of preventive work with the population.
