THE HIV CASE REPORTING SYSTEM IN THE RUSSIAN FEDERATION

Summary:
For effectively tackling the steady progress of HIV it is required to know the true epidemic situation. Meanwhile, in the Russian Federation there is no unified system for HIV epidemiological monitoring.
Thus, the publication aim is to analyze the existing HIV case reporting system in the Russian Federation and Northern-West Region of Russia based on the Rosstat reporting data.
Materials. The analysis was carried out according to Rosstat reporting forms on HIV-infection and tuberculosis: form No. 61 “Information on HIV patient cohorts”, form No. 4 “Information on the results of blood tests for HIV antibodies”, form No. 8 “Information on patients with active tuberculosis” during 2007-2018.
Results and discussion. The difference in newly diagnosed HIV case-notification in 2018 between Rospotrebnadzor and the Ministry of Health of Russia at the whole country level was 33,4%, in the North-West Region of Russia – 41,6%, in Saint-Petersburg – 58,4%, Leningrad and Kaliningrad regions respectively – 34,8% and 23,2%. These discrepancies are due to a different approach to case notification: the Rospotrebnadzor takes into account all newly positive tested cases by immunoblot for antibodies to the HIV, the Ministry of Health — all patients that were newly registered and got under dispensary observation in the AIDS Centers. It is known that not all positively tested individuals for HIV-infection reach and get registered by the AIDS Centers.
Conclusion. In the Russian Federation, including the North-West Region, there are significant contradictions in reporting system of newly detected HIV case-notification between the Rospotrebnadzor and the Ministry of Health of the Russian Federation one. These discrepancies are due to differences in methodological approaches. Only a personalized and unified register, with individual coding of each HIV-positive person, thereby excluding duplicated information, but allowing maximum registration of patients with HIV, will help determine the true HIV epidemic situation in the Russian Federation
