FEATURES OF ORAL FLUID IN AGE-RELATED CHANGES IN THE ORAL CAVITY

Summary:
Actuality. One of the geriatric syndromes and conditions that provoke the development of the frailty syndrome is the development of chronic immune inflammation. Saliva, as well as the oral mucosa, contains key molecules that determine local immunity and provide antimicrobial and antiviral protection. Among these biochemical parameters, it is important to note cytokines, lysozyme, peroxidase, beta-lysines, acid glycoproteins, and others. We have not found any work on the study of these indicators in elderly and senile patients with the frailty syndrome, which led to the relevance of our study. Purpose. to study the biochemical features of age-related mouth syndrome. Material and methods. We examined patients after the age of 70 who had chronic diseases in the compensation stage. The patient groups were divided as follows: a group of patients older than 70 (average age 72.1±1.2 years) without the frailty syndrome (n=61); a group of patients older than 70 (average age 72.4±1.2 years) with the frailty syndrome (n=55). Detection of the frailty syndrome was carried out by the frailty index. Immunological and biochemical tests were performed on the ChemWell 2910 Combi biochemical and immunological analyzer with ChemWell ® software, version 6.1 (Revision A), manufactured by AWARENESS Technology Inc. Cytological smears were prepared by liquid Cytology using the Novoprep automated system (NRS, France). Results. When detecting the level of proinflammatory cytokines, it was proved that in older patients with the frailty syndrome, there is a significant pronounced local inflammatory reaction in the oral cavity, which is expressed in an increase in the level of TNF-alpha by 2.0 times, interleukin -1 – beta by 1.9 times, interleukin -6 by 2.1 times, interleukin – 8 by 1.2 times compared with older patients without the frailty syndrome. The obtained data showed a decrease in local bactericidal, antiviral and antimicrobial protection in older patients with the frailty syndrome, which is probably one of the predictors of polymorbidity accumulation. We also determined that older patients with the frailty syndrome have accelerated cellular aging processes and the risk of developing cancer. Conclusions. In older patients over 70 years of age with the frailty syndrome, chronic immune inflammatory response is pronounced, local antiviral, bactericidal, antihistamine and autoimmune protection is reduced. In addition, we determined that older patients with the frailty syndrome accelerated the processes of cellular aging and the risk of developing cancer.
