Summary:
Relevance. The endocrine system and individual endocrine organs, including the thyroid gland, undergo significant morphofunctional changes with age, like other body systems. Among the conditions that act as predictors of hypothyroidism in older age, pregnancy can strongly affect a woman's hormonal metabolism, especially in the presence of subclinical diseases and thyroid conditions. In this regard, it is interesting to see the further impact of previous hypothyroidism in women who developed this functional state of the thyroid gland during the reproductive period. The aim of the study was to study the development of clinical hypothyroidism in the reproductive period, as a predictor of hormonal problems in old age and old age. Materials and methods. Group with a hypothyroid condition amounted to 265 women, of whom 107 patients occurred postoperative hypothyroidism (diffuse toxic goiter and thyroid cancer history and identified for the first time during this pregnancy, and nodular goiter in history, requiring surgical treatment); at 114 – autoimmune thyroiditis; 14 – congenital hypothyroidism; 23 – polyglandular syndrome and 7 post-radiation hypothyroidism. All patients in this group had hypothyroidism before or during pregnancy. Results and discussion. Analysis of the course of pregnancy in women with hypothyroidism showed that with insufficient secretion of thyroid hormones, pregnancy complications were more common in women, among which the most frequent were anemia – 173 cases (65.3%) and the threat of termination of pregnancy – 163 cases (61.5%). A significant number of pregnancy complications were found in all patients with different types of hypothyroidism, but the more unfavorable course was in women with polyglandular syndrome and congenital hypothyroidism. The study of complications of childbirth and the postpartum period, which are also common in thyroid diseases, was conducted. Conclusion. Thus, it should be noted that when studying hypothyroidism in the elderly and senile women, one should take into account the catamnesis of the disease, including its obstetric part. At the same time, both detected and recorded at earlier stages of thyroid disease have risks to their long-term progression in the post-productive period and will affect both the quality of life and its duration.
