SUBSTANTIATION GERIATRICIAN ROLE IN PROVIDING ORTHOGERIATRIC CARE FOR OSTEOPOROSIS AGAINST THE BACKGROUND OF METABOLIC SYNDROME (SOMATIC, COGNITIVE-PSYCHOLOGICAL AND SOCIAL ASPECTS)

Summary:
Actuality: The problem of research in the field of orthogeriatrics is actualized by the need to create age-oriented protocols of anesthesiological support, search for adequate measures of surgical techniques taking into account the age-related features of osteoarticular system, and develop effective measures of complex postoperative rehabilitation taking into account the polymorbid geriatric background, when involutive changes are superimposed on multiple somatic pathology. Objective: To substantiate the role of a geriatric physician in team orthogeriatric care for injuries associated with osteoporosis and metabolic syndrome on the basis of a clinical and experimental study. Materials and methods: The study included 732 elderly people aged 65 to 74 years, the average age was 70.2+2.3 years, men were 312 people, women-420 people. Depending on the consequences of the falls, the patients were divided into two groups. All patients were examined for somatic and neurological pathology in accordance with the current relevant national clinical guidelines. A comprehensive geriatric assessment was performed using the computer program “Specialized Geriatric Examination". In addition, age discrimination on the Palmore scale and loneliness syndrome on the Russell Ferguson questionnaire were studied. Results: When studying the somatic status, it turned out that the complicated fall syndrome was significantly associated with such nosological forms as arterial hypertension (hypertension), type 2 diabetes mellitus, osteoarthritis, obesity, osteoporosis, chronic heart failure, symptomatic arterial hypertension, anemia, chronic kidney disease, hypothyroidism. The neuropsychiatric status was characterized by a significant presence in patients of pre-dementia cognitive disorders or disorders corresponding to the level of mild and moderate dementia, dizziness syndrome, subdepression or depression of mild and moderate severity, spinal osteochondrosis, sleep disorders. The geriatric status of elderly patients with complicated falls syndrome was characterized by the association of falls syndrome with other geriatric syndromes such as age-related foot syndrome, dyna - /sarcopenia, diabetic foot, malnutrition syndrome and the risk of its development, sensory deficits, incontinence, age-related mouth syndrome. The social status of elderly patients with complicated falls syndrome was characterized by loneliness syndrome, refusal of technical means of rehabilitation, living in an inpatient social institution or living alone at home, the presence of a disability group, medium or high degree of age discrimination, incorrectly selected shoes, smoking, excessive alcohol consumption. Conclusions: Elderly patients with complicated falls syndrome represent a polymorbid group of patients, with diseases and syndrome complexes characterized by metabolic disorders – osteoporosis and metabolic syndrome-being of the greatest importance as a concomitant pathology. The revealed associations of complicated falls syndrome with disorders in the geriatric, somatic, and cognitive-psychological status of elderly patients justify inclusion in the orthogeriatric team of geriatrician.
