GERONTOLOGICAL PREVENTION OF DECREASED RESILIENCE IN PATIENTS WITH CARDIAC ARRHYTHMIAS

Summary:
Actuallity: The resilience is a relatively new concept that characterizes the reserve of the functional and reserve pool in older patients. The resilience consists of certain domains, such as cognitive, motor, psychological, sensory, and bodily. Heart rhythm disorders are a fairly common complication of cardiovascular pathologies, especially among older people. There is a large amount of data in the literature that heart rhythm disorders in older age are significantly associated with a deterioration in geriatric status, which leads to a decrease in the resilience. Objective: To develop gerontological prevention programs for reducing the resilience in patients with cardiac arrhythmias. Material and methods: The basis of the study was the clinical center of the ANO Research Medical Center "Gerontology", Moscow. In our study, we evaluated the prevalence of heart rhythm disorders in patients of different ages. The study sample consisted of 324 patients with cardiac rhythm and conduction disorders in 369 patients, including middle-aged patients-107 people (average age 49.1±1.1 years), elderly patients-145 people (average age 68.4±1.2 years), senile patients– 117 people (average age 74.4±1.3 years). To evaluate the effectiveness of gerontological prevention to improve the resilience in older patients with heart rhythm disorders, we conducted a prospective, controlled, randomized study. The duration of follow-up was 6 months. The effectiveness of the model was evaluated by the dynamics of indicators of biological constants (domains). Results: It was found that the decrease in the cognitive domain occurred by 25.4 %, the motor domain by 25.2%, the psychological domain by 15.1%, the sensory domain by 19.9%, the bodily domain by 14.4%, was due to the development of heart rhythm disorders, which was revealed by correlation analysis (r=+0.897, p
