RESILIENCE AS A NEW OBJECT OF CARDIO-VASCULAR PREVENTION

Summary:
Actuality: The current period of development of health care in the Russian Federation is characterized by the fact that it occurs against the background of an increase in the average life expectancy at birth. In the context of the progressive aging of the population, the prevention of cardiovascular diseases, which occupy the first places in terms of prevalence among older people, is becoming an increasingly urgent problem. Until now, there has been no definition of the point of application of the prevention of decompensation or the development of complications of cardio-vascular pathology from the standpoint of individual viability of a person, which will allow a person of any age to maintain independence, autonomy, the possibility of self-realization and human dignity.
Aim. To consider the resilience as a new object of cardiovascular prevention.
Materials and methods. The study was the clinical centre of the Academy of Postgraduate Education of the Federal State Budgetary Institution of the Russian Federation, Moscow. In the beginning, using the method of continuous selection of patients aged 45 to 74 years, we identified 3 groups differentiated by age and the presence of cardiovascular pathology. During the screening of residual reserves, we focused on identifying 5 biological constants of the resilience, namely, cognitive and psychological statuses, motor, sensory. At the final stage of the study, we proposed and tested measures to optimize preventive care for patients in order to increase the resilience.
Results. The cognitive and psychological domains of the resilience are formed by the predominant activation of pro-inflammatory neuroimmunoendocrine reactions, which are closely related to body mass deficit against the background of malnutrition and anemia, insufficiently corrected arterial hypertension, and atherogenic changes in blood serum with an increase in vascular age in older patients with cardiovascular pathology. A decrease in the motor domain of the resilience is associated with the predominant activation of pro-oxidant processes, and among the main risks of motor disorders is a decrease in the ankle-shoulder index in older patients with cardiovascular pathology. The sensory domain of the resilience is formed mainly due to weakly expressed proinflammatory without a reliable relationship with general somatic changes in older patients with cardiovascular pathology.
Conclusion. To detect a decrease in the biological constants of the resilience, it is important to take into account the features of the functional reserve of the geriatric patient, determine the neuroimmunoendocrine background, and also pay attention to the indicators of biochemical blood analysis, in particular, total cholesterol, low density lipoprotein (LDL), C-reactive protein.
