ORGANIZATION OF SPECIALIZED CARE IN THE CONDITIONS OF CENTRAL DISTRICT AND DISTRICT HOSPITALS

Abstract. In light of the tasks of modernizing primary health care, the construction of new health facilities of "first contact" with their rational placement on the territory of the constituent entities of the Russian Federation and the possibility of receiving specialized care in the immediate vicinity of the place of residence, the task of reviving the dominant role of central district and district hospitals in organization of medical care for the population, optimization of the structure of their hospitals, the list of departments and their capacity in hospitals with different numbers of attached population The purpose . To assess the dynamics of the number of basic bed profiles in central district and district hospitals, indicators of their provision to the population of different constituent entities of the Russian Federation and the level of hospitalization. Methods Data from Rosstat, Ministry of Health of Russia, FGBI "CRIHOI" of the Ministry of Health of Russia. Used analytical, statistical methods and comparative analysis. Results In the period from 2014 to 2019 as a whole across the Russian Federation, the total number of beds in central district and district hospitals decreased by 17.5%. The indicator of the provision of them to the population of the Russian Federation decreased from 19.1 to 15.7 per 10 thousand, the level of hospitalization of the population to central regional and district hospitals decreased from 62.3 per 1000 population in 2014 to 50.0 per 1000 in 2019 (by 19 ,eight%). The average hospital bed capacity decreased by 10.8%. The number of therapeutic beds in central district and district hospitals on average across the Russian Federation decreased by 20.4%, cardiological beds - by 10.3%, neurological - by 12.5%. Surgical beds decreased by 19.2% over the same period. The provision of beds for pregnant women and women in labor in these hospitals has significantly decreased. Discussion The indicator of the provision of the population with therapeutic, surgical and beds for pregnant women and women in labor in district and central district hospitals decreased at a faster rate than the indicator of the provision of beds for highly specialized profiles, with the exception of the provision of ophthalmic beds, most likely due to the formation of a multi-level system of organization of specialized care. Conclusion. In order to ensure the quality and availability of qualified medical care to residents living in rural areas and small towns and cities, it is necessary to stop the reduction in the number and capacity of central district and district hospitals, to develop and approve standard staffing standards for them in accordance with the size of the attached population on the basis of the normative needs for beds of various profiles, taking into account the reasonable structure of the hospital
