EFFICIENCY OF MASS PREVENTIVE EXAMINATIONS OF THE URBAN POPULATION FOR THE PURPOSE OF EARLY DIAGNOSIS OF TUBERCULOSIS IN PRIMARY HEALTH CARE INSTITUTIONS

Summary:
Introduction. The epidemiological situation of tuberculosis in the country is improving. So the question of the advisability of maintaining regular screening examinations of the adult population is important. The purpose of the work is to evaluate the effectiveness of mass fluorographic examinations of the urban adult population. Materials and methods. Studied official statistic data. The indicators of the epidemiological situation and the work on the detection of tuberculosis among the adult population of the Republic of Karelia was compared with federal indicators. A comparative assessment of the efficiency of population surveys for different purposes was carried out according to the primary documentation of the city polyclinic for 2017-2019. Results and discussion. The dynamics of morbidity and mortality from tuberculosis in the Republic of Karelia in 1991-2019, the social structure of cases are similar to those in the country. About 60% of the total number of patients was diagnosed annually among the townspeople. No correlation was found between the dynamics of morbidity, mortality and coverage of the adult with preventive examinations in the republic. The frequency of detection of tuberculosis during preventive examinations in recent years has decreased to 0.3-0.2 per 1000 examined, while in polyclinics they accounted for 82.2% of all fluorographic examinations of the chest organs. Only 25.3% of the examined persons of working age had occupational hazards. In the polyclinic, out of 76,982 studies, 22.6% had a diagnostic goal and 77.3% had a preventive goal. The frequency of detection of tuberculosis during screening was 8.4 times lower than in diagnostic studies: 0.07 and 0.59 per 1000 examined. For 3 years, nobody has been identified among persons without risk factors. Of 34 patients with tuberculosis living in the polyclinic area, 64.7% were detected when seeking medical help, 26.5% during medical examinations (all had risk factors), 8.8% - during examination in the outbreaks. Conclusion. The influence of a decrease in the coverage of the population by fluorographic examinations on the level and dynamics of morbidity and mortality from tuberculosis in the republic has not been established. Low efficiency of detection of tuberculosis among the permanent healthy urban population, which does not have risk factors, was established during mass prophylactic fluorographic examinations.
