Introduction. Cerebrovascular disease are a serious medical and social problem. The problem of prevention of acute disorders of cerebral circulation requires special attention in gerontological practice. This is due to the presence in persons of older age groups of numerous risk factors, comorbid conditions and with a more unfavorable prognosis of each subsequent cerebrovascular event [2]. For today, it is of interest to develop and introduce into practice programs for the secondary prevention of stroke. Within the framework of the pilot project, a unique digital product has been integrated into the health care system of the Belgorod region, one of the main functions of which is personalized secondary prevention of cerebral stroke and control of targeted provision of drugs to the population. Purpose of the study: optimization of the organization of secondary prevention of ischemic strokes by using digital technologies. Method. As part of the study, the structure of the new program in the field of health care "Stroke platform" was analyzed. This is a single online system that combines in a digital environment all stages of treatment of a patient with a stroke and control of targeted provision of the patient with medications. The program includes several blocks: neuroimaging block register of patients with stroke, the register secondary prevention routing desk, statistics and analytics. Results. The innovative search engine allows to use a variety of indicators for analysis. One of the important results of digitalization is the predictable optimization of the organization of secondary prevention of ischemic strokes. Obtaining reliable statistical data will make it possible to adequately plan the volume of medical care to the population, reduce morbidity, disability, and improve the quality and life expectancy of the population [7]. Conclusions. The stroke platform is an example of successful integration and information technology in healthcare. Such online systems undoubtedly increase the level of secondary prevention of cardiovascular events in the region, making them continuous, consistent, personalized and convenient for managing patients of older age groups.
