ORGANIZATION OF PALLIATIVE MEDICAL CARE FOR CHILDREN IN THE MOSCOW REGION

Summary:
palliative care for children with incurable diseases is one of the priority tasks of health management bodies. The purpose of the study to study the organization of palliative care for terminally ill children of the Moscow region. Materials and methods: Medical organizations providing palliative medical care to children of the Moscow region from 2017-2020. The basis for the study is the data of the forms of federal statistical observation (FSN): FSN No. 12 "Information on the number of diseases registered in patients living in the service area of a medical organization", FSN No. 14 "Information on the activities of departments of a medical organization providing medical care in inpatient conditions" and FSN No. 30 "Information about a medical organization". Data from the register of children in need of PMC. Results: the study revealed that in the region since 2018, there have been significant changes in the organization of the provision of medical PMC in outpatient, inpatient conditions and at home. A coordination center was created on the basis of the Moscow Regional Hospice (for Children) on the basis of the Moscow Regional Hospice (for Children), which made it possible to form a regional register of children, which included 459 children according to the results of the work of 2020, and 481 children in the first quarter of 2021. Routing begins after the conclusion of the medical commission on the recognition of the child in need of PMC and continues throughout the course of the disease. The purposeful work on the organization of the registration of children made it possible to determine the need for outpatient and hospital care. The number of inpatient beds of PP in the region from 2018 to 2020 increased by 34.3%, and the employment of beds by 33.9%. Specialists are systematically trained according to the program of providing PMC, using telemedicine technologies, an accessible ergonomic environment is organized for patients in homes and public places. 6 visiting brigades were formed to provide visiting patronage service, the number of patronage visits by the end of 2020 increased by 2.5 times to the level of 2019. Children are helped by multidisciplinary teams. Home care is organized with the possibility of conducting laboratory and diagnostic studies. Conclusion: the implemented medical and organizational measures made it possible to achieve the following results: the provision of palliative beds was 0.3 beds per 10,000 children's population; the number of outpatient visits to specialists of various specialties has increased to 154.7 per 10,000 children's population; the provision of visiting visiting teams of PMC is equal to 0.95 per 50,000 children's population, and the share of visits by visiting visiting teams at home, in the total number of visits of PMC to the children's population has increased to 75%. The complex of medical interventions aimed at getting rid of pain and relieving other severe manifestations of the disease significantly affected the reduction of physical, psychological, social and spiritual suffering.
