Abstract. Strokes and their sequalae are one of the reasons for the development of negative medical, demographic and socio-economic consequences. A group of special risk of negative consequences formation are males of working age, the rural population, as well as persons with the stqualae of a stroke. A progressive increase in the frequency of deaths not only from acute types of the disease, but also its sequalae require both the improvement and optimization of the existing forms of medical care for patients with this pathology, and the development of new organizational measures aimed at preventing premature mortality of the population from acute strokes and their sequalae, especially in relation to the harsh conditions of the Arkhangelsk region. The main goal of our study was to develop measures to reduce mortality from acute cerebrovascular accidents and their consequences in the Arkhangelsk region. Materials and methods. A continuous analysis of 13759 medical certificates of deaths from strokes and their sequalae in the Arkhangelsk region was carried out. Taking into account the data obtained, an analysis of the loss of life potential in this disease in the Arkhangelsk region for 2011–2019 was carried out. The effectiveness of the targeted programs to combat diseases of the cardiovascular system was determined in terms of premature mortality of the population from the studied pathology, and a set of organizational measures was developed to improve the provision of medical care in strokes and their sequalae, aimed at reducing premature mortality from this group of conditions in Arkhangelsk region. Results. The overall dynamics of the rate of loss of life potential decreased by 27.0% among men and 46.5% among women. The effectiveness of federal and regional targeted programs to combat diseases of the cardiovascular system in the Arkhangelsk region is represented by a decrease in the rate of loss of life potential from strokes and their sequalae in 2019 compared to 2011 among the male population by 11269 person-years, which is equivalent to 140 , 9 males and females per 18,029 person-years, which is equivalent to the life of 218.5 women. Conclusion. Implementation and implementation in practical health care of a set of developed organizational measures to improve the provision of medical care for acute disorders of strokes and their sequalae based on the results of the study, aimed at reducing mortality from this group of conditions, will create conditions for increasing the survival rate and quality of life of patients with this pathology and will allow to achieve an improvement in the population health of the residents of the Arkhangelsk region.
