ORGANIZATION OF EMERGENCY IN THE REPUBLIC OF CRIMEA AS A NEW STAGE OF IMPROVING THE SYSTEM OF EMERGENCY MEDICAL CARE IN THE CONDITIONS OF THE HOSPITAL STAGE

Summary:
Introduction. The activity of emergency medical teams in the conditions of an epidemiological situation associated with a new coronavirus infection is associated with the need to carry out frequent unscheduled visits to transport patients with a new coronavirus infection COVID-19, as well as those taken to the Observatory, especially at night, mass collection of biological material for laboratory diagnostics from a certain contingent of people, which requires solving new ways in the organization of emergency, including emergency specialized medical care. Another feature of the provision of emergency services in the Republic of Crimea, including emergency specialized medical care, is the lack of inpatient emergency departments.
The purpose of the study was substantiation of the new format of work of inpatient emergency departments in the Еmergency format in order to promptly regulate the flow of patients in the provision of emergency medical care at the hospital stage.
Materials and methods. Analysis of statistical indicators in the work of emergency medical teams at the stage of hospitalization of patients in need of emergency medical care, taking into account the availability and quality of specialized medical care services.
Results and discussion. Against the background of an increase in the delay time of emergency medical teams in the emergency departments of medical organizations of the Republic of Crimea by 1.8 times, due to the lack of inpatient emergency departments in medical organizations of the region, an increase in the number of patients delivered by emergency medical teams to emergency departments in 2018 was established from 67.7 to 75.9 in 2019 and 76.3 in 2020. In addition, the level of medical care in the Republic of Crimea with a shortage of specialists of a "narrow" profile, a reduction in the bed Fund is characterized by a decrease in the rate of hospitalization in 2018 from 78.3% in 2018, to 77.9% in 2019, and 76.8% in 2020, respectively. As a result, to ensure the availability and quality of specialized medical care services, an increase in the number of applications for air ambulance services was noted (about more than 210 - 268 evacuations over the past three years in a row). The identified problematic trends in hospitalization, in modern conditions, cause the reformatting of existing patient routing schemes for hospitalization and increasing the readiness of medical organizations to receive and select emergency patients for specialized treatment.
Conclusion. The optimal implementation of promising targets of improving emergency, including emergency specialized medical care provided by the State program of the Russian Federation "Development of health" with a number of measures to improve the hospital phase of emergency medical care aimed at improving the availability and quality of medical care, is the organization of health care organizations, inpatient wards, emergency medical care on the type of Еmergency.
