Summary:
The problem of organizing affordable and high-quality medical care remains unresolved to this day. The main indicator of the nation's health - "Life expectancy" is declining. The so-called “optimization” has aggravated the situation in healthcare. Despite the measures taken by the country's leadership: maternity capital, preferential mortgages and others, the "demographic hole" is not only not decreasing, but is increasing every year. The forecasts published by Rosstat are pessimistic. In seven years, if urgent cardinal measures are not taken, the population of the country will decrease annually by 600 thousand people, irreversible processes of self-destruction of the nation will begin. The situation is aggravated by the coronavirus; the incidence is growing in most regions of the country. In the new economic conditions, monetary remuneration is becoming a preferred form of incentive and an effective means of managing the quality of medical care in a polyclinic, provided that the amount of cash payments is directly dependent on the assessment of the quality of care given by patients. Topicality. The decline in the availability and quality of primary health care is now a consequence of the fact that the amount of cash payments to patients is directly linked to the quality of patient care. Work goals/tasks. To make a correction in the method of evaluation and management of the quality of medical care at the doctor's appointment in the clinic, based on the use of the results of sample surveys of patients, directly affecting the amount of additional monetary remuneration, Outcomes. Unfavorable health-demographic trends, increasing morbidity, high mortality rates from circulatory diseases, cancers, and the inadequacy of primary health care organizations are putting forward new demands for better accessibility and quality of care without associating them with the amount of cash payments to patients. Material and methods. Material: Analysis of the responses and opinions of the participants of the department of health and public health of the RMANPO and health workers of the Moscow system in assessing the current system of health care organization to the population. Methods: Analytical, comparative analysis, statistical. Findings. According to the results of the study, the following conclusions were drawn: the proposed correction in the method of evaluation and management of the quality of medical care at the clinic, based on the use of the results of selective surveys of patients directly affecting the amount of additional monetary reward, confirmed its effectiveness.
