TUBERCULOSIS IN PENITENTIAL INSTITUTIONS OF THE RUSSIAN FEDERATION IN COMPARISON WITH THE COUNTRIES OF THE EUROPEAN REGION OF WHO AND THE WORLD AT THE BORDER OF THE THIRD DECADE OF THE XXI CENTURY

Summary:
The relevance of the study is due to the need to conduct a regular comparative analysis of the epidemic situation of tuberculosis and the results of treatment of patients in order to identify segments of the TB care system that need to be improved. Methods. Analyzed the data of the official statistical materials of the European region of WHO and eCDC, the data of official statistics. TB incidence rate was calculated as the ratio of the sum of new TB cases and relapses to the average number of prisoners. The proportions of treatment outcomes calculated as the percentage of the number of patients with the treatment outcome to the number of patients in the 2019 cohort. Results. The Russian Federation is in fourth place in the frequency of incidents of tuberculosis among prisoners (1215.3) among the countries of the WHO European Region. In 2020, the decline in the incidence rate accelerated to 10.4%. The Russian Federation has the highest proportion of incidents of tuberculosis in prisons in the WHO European Region - 8.9%. The relative risk of an incident in the prisons of the Russian Federation is also the highest - 24.2. This is probably due to the high prevalence of HIV in prisoners. Russia is one of the three countries with the lowest proportion of treatment success rate (52.4%), leading in the proportion of treatment failures (10.3%). Not evaluated treatment outcomes are a serious problem (29.6%). Conclusion. The epidemic situation with tuberculosis in prisons is worse than among the general population. This is confirmed by both the high incidence of TB and the high relative risk of a TB incident. Despite the pronounced positive dynamics, the Russian Federation still ranks first in the proportion of incidents of tuberculosis detected in prisons, as well as the relative risk of tuberculosis among prisoners compared to the population. A weak point in the TB care system for prisoners in Russia is the low rate of treatment success associated with a high rate of most unfavorable outcomes (excluding loss to follow-up). Particularly alarming is the high proportion of treatment failures against the background of a relatively low proportion of cultural confirmation of the diagnosis. It is necessary to pay close attention to the quality of microbiological diagnosis of tuberculosis and take measures to improve it.
