MPROVING THE OPERATION OF THE OPERATING UNIT OF A MULTIDISCIPLINARY HOSPITAL

Summary:
Introduction. The effective operation of the hospital's operating unit is an urgent task in the healthcare system that requires managerial and organizational decisions. Improving the activity of the operating unit in many ways allows us to solve the problems of large multidisciplinary hospitals of surgical profile.
Purpose: to develop organizational measures to improve the work of the operating unit of the city multidisciplinary hospital and evaluate their effectiveness.
Materials and methods. The research base is Samara City Clinical Hospital No. 1 named after N.I. Pirogov. The performance indicators of the operating unit in 2016 and 2021 were developed and studied using the form 008/u "Journal of surgical interventions in the hospital", the database of the automated information system "Hospital". The operating room efficiency coefficient has been developed as the ratio of operating room workload to staff working time per shift (in %).
Results and discussions. Organizational measures have been developed and implemented to improve the activities of the operating unit of a multidisciplinary hospital, taking into account the pre-, intra- and postoperative period of patient management. A computer program "Operating unit management program of a multidisciplinary hospital" has been developed and implemented, which allows you to automatically plan and approve the schedule of operations, manage all stages of the operation unit. The introduction of new organizational measures made it possible to increase the number of operations performed by 7.7% from 1643.3 operations per month (2016) to 1770.6 operations per month (2021), optimize the staffing of the Department of anesthesiology-intensive care and the operating unit, maintain and expand the profile structure of operations with an increase in the number of operations performed as part of the provision of high-tech medical care by 79.5%.
Conclusions. The introduction of organizational measures allowed to reduce the average time of the operating cycle (including the stage of patient transportation with simultaneous preparation of the operating room, the stage of anesthesia, the stage of the operation itself, the stage of cleaning and disinfection of the operating room) from 132 minutes in 2016 to 93 minutes in 2021 (by 29.5%) by reducing the duration of the stage of patient transportation and preparation of the operating room from 45 minutes in 2016 to 12 minutes in 2021, to reduce the average time interval between operating cycles from 49 minutes in 2016. up to 2 minutes in 2021, to increase the ratio of planned and performed operations to 99.8% and increase the operational efficiency ratio from 53.2% to 85.3%.
