PROBLEMS OF ORGANIZING MEDICAL CARE FOR THE ELDERLY IN THE RUSSIAN FEDERATION

Summary:
Introduction. The article provides a bibliographic review of last 10 years-scientific articles about the current state of health care from the point of view of targeting elderly patients. In Russia, since the beginning of the 60s of the XX century, the share of the elderly in the structure of society has been increasing every year. So, in 2010, according to the Federal State Statistics Service, the share of people over 60 years of age was 18.2 % of the total population, and in 2020, the share of the elderly among the population was 22.4 %. There are regional features of this problem, the aging process of the population is more pronounced in the European part of Russia. Aging is accompanied by a physiological transformation of the body, a change in mental status, a decrease in productivity and working capacity. An elderly patient differs from a patient of working age, which is due to the physiological characteristics of aging patients. In the future, the age structure of the Russian population will also change, following modern demographic trends: an increase in the share of older people in the total population, a decrease in the share of people of working age and younger ages.
Objective: To study the organization of geriatric care in the Russian Federation with regional trends and peculiarities.
Methods and materials. The article provides a bibliographic review on the topic of the current state of healthcare in terms of targeting elderly patients using open statistical sources, scientific articles and descriptions of healthcare practices.
Results. When organizing outpatient care, it is necessary to take into account the structure of the treatment of elderly patients: for example, every third elderly patient does not apply for the disease, but for preventive purposes. There is also a problem of insufficient provision of medical services to elderly patients. This may be the reason for the reorganization of the appointments of district doctors.
Issues of inpatient care rest on the multimorbidity of elderly patients and the frequent need for general care. For this purpose, geriatric hospitals are provided within the framework of the "Older Generation" program, the model of which has not been determined in regions of Russia yet. The model is intended to provide medical and social care in such institutions in combination with social, labor and medical rehabilitation.
The geriatric service cannot be built without the participation of educational institutions-universities, the program of each specialization must include an introductory course in geriatrics, and there is also a need to train qualified nurses to care for the elderly.
Conclusions. The need to organize a geriatric service is due to the continuous demographic changes and age-related restructuring of society, which requires the integration of all possible efforts – medical, social, educational, scientific and organizational.
