BIOMOLECULAR ASPECTS OF ACCELERATED AGING WITHIN THE PATHOPHYSIOLOGICAL GERONTOLOGICAL CONTINUUM

Summary:
Introduction. The pathophysiological gerontological continuum implies the presence of causal relationships in the combination of geriatric syndrome with age-associated and sociomatic pathology. Geriatric syndromes include malinitration syndrome, sarcopenia, senile asthenia, falls, etc. Sarcopenia is a loss of muscle strength and mass in older adults and is a common disease and is also associated with several adverse health outcomes. Due to the impact of sarcopenia on quality of life, disability and mortality, greater awareness is needed in order to correctly identify this condition and in particular its biological immunoendocrine markers. Aim: to characterize molecular changes in the pathophysiological gerontological continuum in sarcopenia. Materials and methods: the study included 402 elderly people who were divided into three groups depending on the presence/absence of arterial hypertension and sarcopenia: 116 people entered the group of people with arterial hypertension without sarcopenia; in the group of people with hypertension and sarcopenia - 174 people; in the group of people without arterial hypertension and without sarcopenia (control) - 112 people. To determine the degree of oxidative status, glutathione peroxidase parameters in erythrocytes, free fatty acids, lipid peroxidation, and general antioxidant status were studied. Levels of somatotropic hormone, insulin-like growth factor, testosterone, progesterone, estradiol in blood serum were also investigated by standard methods. The results of the study: it was found that the pathophysiological gerontological continuum in sarcopenia is characterized by pronounced pro-oxidant changes in the glutathione peroxidase level in erythrocytes and in the content of non-esterified fatty acids in blood serum, while a reliable increase in glutathione peroxidase in erythrocytes is observed 2.2 times and reliable. It has been proved that in sarcopenia and arterial hypertension there are common pathophysiological mechanisms in the form of oxidative stress. Oxidative stress activity is characterized by increased levels of glutathione peroxidase in erythrocytes, non-esterified fatty acids in blood serum, which is aggravated by neuroimmunoendocrine imbalance in the form of reduced levels of testosterone, progesterone, somatostatin and insulin-like growth factor. Discussion: The findings demonstrate an unambiguous trend towards neuroimmunoendocrine and metabolic imbalance as age-associated pathology progresses. This trend is consistent with the data of the scientific literature showing the negative impact of lipid peroxidation products on quality of life. It has been shown that free radicals contribute to the acceleration of aging processes and, as a result, the early development of geriatric syndromes. Many experts recognize the protective role of steroid hormones in preventing the development of both age-associated and sociomatic pathology. In addition, the above data prove that a decrease in serum insulin-like growth factor levels can serve as a predictor of sarcopenia development in both men and older women within the pathophysiological gerontological continuum.
