TUBERCULOSIS IN THE RUSSIAN PENAL SYSTEM IN 2021

Summary:
Introduction. Monitoring the epidemic situation of tuberculosis in penitentiary institutions is an actual task. Its results are analyzed not only at the national, but also at the international level. The purpose of the study is to monitor the epidemic situation of tuberculosis and the results of the implementation of anti-tuberculosis measures in Russian prisons. Methods. We used official statistics, standard methods of statistical processing of information. Results. The number of new TB cases in 2016–2021 decreased from 277 to 576 per 100,000, incl. in detention centers from 1416 to 1040, and in correctional facilities from 642 to 350 per 100,000. The relapse rate of TB decreased from 357.8 to 318.8; the frequency of early relapses is on average 11.6%. The mortality rate from tuberculosis has decreased by 2021 to 4.9 per 100,000. The prevalence of tuberculosis in 2021 has decreased to 2078.5 per 100,000. HIV prevalence in 2018–2021 stabilized at the level of 10512.8–10729.8 per 100,000, and the comorbidity (TB/HIV) rate at the level of 32.3–33.4% of the number of patients with tuberculosis. The prevalence of multidrug-resistant tuberculosis (MDR) decreased to 518.7 per 100,000 with an increase in primary MDR to 33.6%. Treatment success rate of pulmonary TB patients increased to 57.9%, while the proportion of non-evaluated treatment outcomes decreased to 25.8%. Conclusions. From 2016 to 2021, there has been a synchronous decrease in the incidence in pre-trial detention centers and correctional facilities by 24%. TB relapse rate is decreasing at a slower - 10.9%. This corresponds to the problem of adherence to TB treatment and its controllability in Russia. The COVID-19 pandemic did not have an impact on the epidemic situation of tuberculosis in prisons. There are prerequisites for meeting the target value of the End TB Strategy in 2025. Mortality from tuberculosis has reached its lowest level. It is expedient to analyze the causes of mortality in patients with tuberculosis from other causes. The increase in coverage of antiretroviral therapy has led to a decrease in the incidence of tuberculosis and stabilization of the proportion of TB/HIV at the level of 1/3 of the number of TB patients. There is an improvement in treatment outcomes, primarily due to the introduction of recommendations to improve the monitoring of treatment of patients with tuberculosis.
