PROGNOSTIC FACTORS FOR THE FORMATION OF SEVERE ATOPIC DERMATITIS IN ADOLESCENT CHILDREN

Relevance. Predicting the course of atopic dermatitis in children is an urgent and little-studied problem. Currently, a large number of studies are devoted to the study of risk factors for the manifestation and development of atopic dermatitis, but not the prediction of the formation of a severe course of the disease. At the same time, adequate and personalized rehabilitation of a child with this pathology is impossible without the timely identification of factors and their combinations that can influence the course of the disease. This determines the need to search for additional criteria to predict the course of atopic dermatitis in adolescence. The aim of the study was to study the prognostic risk factors for the formation of a severe course of atopic dermatitis in adolescent children in order to optimize dispensary observation. Materials and methods. The observation group consisted of 350 children aged 13-17 with atopic dermatitis, comparison - 350 children of the same age of health groups I-II without allergic diseases. An analysis of the medical documents of children was carried out, a survey of parents was carried out. Main results. As a result of the study and analysis of biomedical and socio-hygienic aspects in adolescent children with AD, depending on the severity of the course of the disease, we have identified the factors that are most often present in the group of adolescents with severe course. It should be noted that both biological and socio-hygienic factors influenced the severity of the disease. According to our data, 14 factors were significant prognostically unfavorable factors in the formation of a severe course of AD by the teenage period of a child’s life, of which: 7 are “controllable” (amenable to complete or relative correction) and 7 are “uncontrollable”. Identified in adolescent children with AD, significant factors in the formation of a severe course of the disease made it possible to develop an algorithm for predicting the course of AD and supplement the monitoring program for these children. The development of an individual preventive and (or) rehabilitation program should be aimed at reducing the impact of the identified factors The procedure for the formation of an individual preventive (rehabilitation) program for adolescent children with atopic dermatitis has been drawn up. A set of measures has been proposed in three directions: “controlled” prognostic factors, possible causes of factors, ways and methods to reduce the influence of prognostic factors on the course of atopic dermatitis. Conclusion. Individual prediction of the course of atopic dermatitis by adolescence of the child will help prevent the formation of severe forms of the disease.
