Relevance. The personnel problem in state medical organizations for many years continues to be one of the most urgent. The solution of this issue requires a parallel determination of the real need for medical personnel. The issues of determining the need for medical workers have been the subject of study by many researchers, however, there are not enough works devoted to solving the problem of providing secondary medical personnel. In this regard, it is most expedient to evaluate the applied methods of calculating needs and their further improvement, taking into account modern conditions of medical care and the changing organizational structure of medical organizations. The improvement of existing methods for determining the need for personnel, taking into account modern features in healthcare, will undoubtedly increase the level of practical application of calculation methods. The purpose of the work. Development of scientifically based proposals to improve the methodology for calculating the need for specialists with secondary medical education. Results and discussions. Currently, in order to determine the need for secondary medical personnel, the subjects of the Russian Federation use the methodology approved by the Order of the Ministry of Health of the Russian Federation No. 973 dated 29.11.2019 "On approval of the methodology for calculating the need for specialists with secondary medical education". According to the results of a sociological survey of specialists in the field of healthcare organization, the low practical applicability of the above-mentioned Methodology by the subjects of the Russian Federation was revealed, which is primarily due to the need to improve it taking into account modern requirements of medical care. Thus, 49.3± 5.52% use the Methodology only to fulfill a separate order of the Ministry of Health of the Russian Federation, and in 76.9± 4.66% of cases it was found that the Methodology requires revision. The improvement of the Methodology will increase the level of application and make it possible to plan the use of the human resources of secondary medical personnel at the level of a specific region of the Russian Federation or a medical organization. Our proposed method of calculating the need for average medical personnel is based on a differentiated modeling of the calculation taking into account the normatively determined method. To determine the need for secondary medical personnel providing medical care in outpatient settings, the following were used: estimated need for medical personnel, standards for the number of secondary medical personnel, planned volume of activities (visits), organizational structure of medical organizations and the number of the population served. Conclusions. A feature of the improved methodology for calculating the need for average medical personnel is the application of a differentiated approach to each position of average medical personnel, a "vertical" calculation method (from the level of the medical organization), taking into account the organizational characteristics of the medical organization, moving away from average values to objective ones. The developed approach is not a theoretical one, but a working tool for practical application.
