MORTALITY FROM MAJOR INFECTIOUS AND PARASITIC DISEASES: DISEASES CAUSED BY HIV, TUBERCULOSIS AND PARENTERAL VIRAL HEPATITIS IN THE RUSSIAN FEDERATION AND ITS DYNAMICS IN 2015-2020

Summary:
Objective: to study age, gender and geographical features, as well as the dynamics of mortality from the most significant infectious and parasitic diseases (IPD): HIV-related diseases (HIV), tuberculosis and parenteral viral hepatitis (PVH) in 2015-2020. Methods: official data on the causes of death by sex, age and geography 2015-2020 have been studied. Results. Despite the fact that the share of IPD in the structure of causes of death in the period under review averaged 1.8%, their contribution to the total number of years of life lost (YLL) amounted to 11.1%, and to the structure of labor potential losses – 10.8%. In the structure of mortality from IPD, there is an increase in the proportion of HIV (up to 61.3% by 2020), PVH (up to 6.6%) with a decrease in the proportion of tuberculosis (up to 22.7%) and other IPD (up to 9.4%). The TB/HIV in the structure of IPD exceeds 21.3%. The gender structure of IPD is dominated by men; in 2015-2020, the share of women in the structure of mortality from PVH has not changed (39.3%), from HIV, increased from 31.2% to 34.0%, from tuberculosis increased from 20.8% to 22.5%, from other IPD increased from 44.3% to 47.9%. The maximum number YLL from IPD was noted in 2017 – 960482 years; by 2020, the it decreased to 752780. In the structure of YLL by 2020, there is an increase in HIV to 70.5%, as well as PVH to 5.1%, with a decrease in the proportion of tuberculosis to 16.2% and other IPD to 8.1%. There is an increase in the age of deceased HIV, tuberculosis and less pronounced in PVH. The number of lost years of labor potential in 2015-2019 changed little (from 509486 in 2018 to 549187 in 2016), decreasing to 549187 in 2020. The mortality rate of the urban population from HIV and PVH is higher than that of the rural population, but the mortality rate from tuberculosis is higher in the rural population. Mortality from IPD varies by Federal Districts: in 2020 for HIV – 5 times, tuberculosis – 4.7 times, PVH – 2.7 times. Conclusion. Mortality from IPD has age, gender and regional specifics, and also changes in dynamics. The damage caused by IPD in YLL and labor potential lost significantly exceeds their contribution to the structure of mortality. Periodic analysis of mortality from IPD is advisable.
