REHABILITATION AND REVERSIBILITY OF BIOLOGICAL AGE IN THE ASPECT OF PATHOPHYSIOLOGICAL GERONTOLOGICAL CONTINUUM

Summary:
Introduction. The pathophysiological gerontological continuum denotes the presence of cause-and-effect relationships in the combination of geriatric syndrome with generic disease. The most common geriatric syndromes are sarcopenia, which is characterized by progressive loss of muscle mass in elderly and senile people.
Purpose: to develop and substantiate targeted rehabilitation programs in the combination of hypertension and sarcopenia.
Materials and methods: To assess the effectiveness of targeted rehabilitation programs (CRP), a prospective, controlled study was conducted, during which two groups of elderly patients with a combination of hypertension and sarcopenia were formed by random selection: control (received standard prevention and rehabilitation programs) and basic (these people received CRP). Study design agreed with local ethics committee, patients gave informed consent.
Results: The implementation of CRP demonstrated that after 3 months, indicators reflecting the inflamaging processes improved significantly. Thus, the level of tumor necrosis factor decreased by 27% in the main group, while in the control group the studied indicator did not significantly decrease; at 3 months, the level of tumor necrosis factor in the main group was 24.5% lower than in the control group. The level of interleukin 1 beta in the main group decreased by 33.8%, while in the control group the studied indicator did not significantly decrease; at 3 months, the level of interleukin 1 beta in the main group was 34.7% lower than in the control group. The level of anti-inflammatory interleukin 4 increased by 44.2% in the main group, while in the control group the studied indicator did not change significantly, after 3 months the level of interleukin 4 in the main group was 47% higher than in the control group.
Discussion: Inclusion of CRP in the complex of preventive and rehabilitation measures for sarcopenia of nutritional support, which is aimed at managing the most important components of PGC, namely, suppressing inflamagging processes, reducing the severity of oxidative stress, activating metabolism, suppressing the production of pathologically active biological molecules by adipocytes, leads at the organ level to optimization of muscle function parameters, and at the organism level - improvement of gait speed, movement, resistance and health-related quality of life indicators. The biological effectiveness of the CRP for the development of sarcopenia and rehabilitation was confirmed by the protection of oxidative status and a decrease in the severity of inflamaging.
Conclusions: Thus, a targeted rehabilitation program based on nutritional support of standard methods for the prevention and correction of the pathophysiological gerontological continuum has been developed and substantiated. It was established that the developed program is able to influence the indicators characterizing biological age: cytokine profile, oxidative status.
