IMPROVING THE ORGANIZATION OF PRIMARY HEALTH CARE IN THE SVERDLOVSK REGION

Summary:
Abstract. In the context of the introduction of the New Model of a Medical Organization Providing Primary Health Care, as well as against the backdrop of the spread of a new coronavirus infection, it is necessary to assess the level of outpatient care at the regional level in order to take additional measures to improve the work of the healthcare system.
Purpose - based on a comprehensive assessment of the work of medical organizations providing primary health care on an outpatient basis, at the regional level, propose directions for improving primary health care.
Materials and methods. An assessment was made of indicators characterizing the organization of primary health care based on data from Rosstat, the Ministry of Health of Russia, FRIHOI of MoH of the RF, MIAC, TFCMI SR and insurance medical organizations of the Sverdlovsk Region. Analytical and statistical methods were used.
Results. The leading problems in the organization of primary health care (lack of staff, excess of the recommended number of attached population in the areas, an increase in the number of complaints from patients) are identified and the main ways to solve them are analyzed: opening pre-medical appointments, increasing the number of departments and emergency rooms, introducing remote recording for admission to specialists, lean technologies, the development of the institution of insurance representatives.
Discussion. Most of the problems in the development of primary health care are similar in many regions of the Russian Federation. When developing directions for optimization, it is necessary to take into account the territorial features of the region, the assessment of resources and the features of the development of the primary health care system. To make management decisions, it is necessary to conduct a comprehensive analysis and study the estimated indicators characterizing the availability and quality of outpatient care organization. It is necessary to develop and implement regional standards, regulations, work algorithms to improve the areas of work of the primary link, including in the context of the spread of a new coronavirus infection.
Conclusions. The problems of quality and accessibility of primary health care remain relevant for many years and determine the adoption of systemic management decisions to improve the organization of medical care in an outpatient setting. The key areas for improving and optimizing the system are: the efficient use of human resources, the formation of the necessary competencies of employees in a changing healthcare system, the transition to a patient-centered outpatient care system based on the introduction of modern management technologies.
