Conduction. Regional programs of modernization of system of healthcare are directed on optimization of use of three-level system of rendering of medical aid. In the given system the priority role is allocated to regional medical institutions which are called up to render special-purpose including high-tech medical care. These organizations represent large versatile establishments in which the significant material and personnel potential of all branch is concentrated. Goal. The analysis of the importance of regional establishments in increase of availability of high-tech medical care for residents of the Astrakhan area. Materials and methods. Research covered the period with 2010 for 2021. 11762 cases of rendering of high-tech medical care have been analyses by all regional establishments of the Astrakhan region. Results. Carried out research has shown, that the quantity of structures rendered by regional establishments from one in 2010 up to 14 in 2021 has increased. On their fraction of Astrakhan is necessary 3/4 of all executed cases of high-tech types of the help the medical organizations. Thus about half (48,6 %) the Alexandro-Mariinsky regional clinical hospital has executed, 18,7 more % - a regional oncological clinic and 8,5 % - regional children's clinical hospital. In dynamics of volumes increase of rendering of the given medical aid almost in 19 times, with a maximum fallen 2017 and made 1382 cases is traced. The greatest volume on the average for all studied period, appears in such directions, as cardiovascular surgery, traumatology and orthopedy, oncology and neonatology. Discussion. Regional establishments play an essential role in rendering special-purpose medical aid as they have more possibilities for introduction of currently available methods of diagnostics and improvement of quality and efficiency of treatment. In these establishments there are all conditions for rendering high-tech types of medical care that assists its approximation for the population of region. Conclusion. As has shown, carried out research, for increase of availability of high-tech medical care is necessary to optimize use of regional resources of healthcare.
