PATIENT SATISFACTION WITH AMBULATORY MEDICAL CARE

Summary:
Introduction. The provision of medical care in outpatient settings, that is, within the framework of primary health care, is one of the priorities of domestic healthcare, therefore, the study of patient satisfaction with care is an important mechanism for identifying problems and making organizational and managerial decisions.
Purpose: to characterize patient satisfaction with medical care in outpatient settings.
Materials and methods. The study was conducted on the basis of the state budgetary healthcare institution of the Samara region (GBUZ SO) "Novokuibyshevskaya Central City Hospital" (NCGB) – a large medical organization serving the population of Novokuibyshevsk of the Samara region with a population of 100 thousand inhabitants. NCGB is a united city hospital, which has a polyclinic department and a hospital. As of the beginning of 2022, the capacity of the polyclinic for adults was 1,150 visits per shift, the number of attached adult population was 84.2 thousand. a person who was provided with medical care in an outpatient setting by 153 doctors. A survey of 625 patients aged 18 years and older was conducted using a specially developed "Questionnaire on the study of patient satisfaction with the provision of medical care in outpatient settings". Sociological, analytical and statistical research methods were used.
Results and discussion. The medical and social characteristics of adult patients were obtained, as a result of which it was determined that at an average age of 49.8 years, women (62.7%), persons with full secondary education (65.0%), married (54.4%), with a satisfactory financial situation (68.2%), working (63.5%) predominate among patients. A satisfactory state of health is noted by 54.7% of patients with an average score on a five-point health scale of 3.3 points). For 50.4% of patients, the reason for applying to an outpatient unit is a disease, most patients receive medical care at the expense of CHI (91.9%), 46.9% of patients applied to a medical organization directly through the registry. Only 20.3% of the surveyed patients got to the doctor on the day of their visit, the average waiting time for a doctor's appointment was 4.7 days. At the same time, 42.1% of patients experienced some difficulties when making an appointment with a doctor.
Conclusions. There is a rather low satisfaction of patients with various aspects of providing medical care in outpatient settings. Patients are more or less dissatisfied with: the work of the registry (42.4% with an average satisfaction level of 3.7 points), the conditions of medical care (44.3%, satisfaction level - 3.7 points), the completeness of information about the outpatient service (45.1%, 3.7 points), the provision of medical care by a district doctor (42.1%, 3.8 points), the provision of medical care by a specialist doctor (33.2%, 4.0 points), the provision of medical care by secondary medical personnel (29.6%, 4.1 points), the provision of medical care in outpatient settings in general (40.7%, 3.8 points). The identified problems in ensuring patient satisfaction with the provision of medical care in outpatient settings require organizational measures.
