COMPARING MORBIDITY AND GERIATRIC SYNDROMES BETWEEN MOBILITY-IMPAIRED AND BEDRIDDEN PATIENTS IN HOME MEDICAL CARE

Summary:
Introduction: Due to the increase in life expectancy, as well as the number of elderly and senile people, the health care system is facing an increasing number of geriatric patients. Some of them are not able to visit medical organizations on their own and prefer to live at home, being patients of the patronage service. To improve the quality of patronage care, we consider the structuring of this heterogeneous cohort of patients to be targeted. Purpose: to conduct a comparative analysis of the structure of morbidity and geriatric syndromes in patients with limited mobility and bedridden. Materials and methods: The study included 450 (76% of women) patients with limited mobility and bedridden patients under the supervision of the patronage service of GBUZ "Diagnostic Center No. 3 DZM" from March 2020 to March 2021. The incidence of chronic non-communicable diseases (CNID) and the prevalence of geriatric syndromes were assessed based on a comprehensive geriatric assessment: vision and hearing, the presence of pain syndrome, urinary incontinence, nutritional status, the number of hospitalizations over the past year, the number of drugs taken and the most common groups, anthropometric data, the level of systolic and diastolic blood pressure. A number of laboratory tests were performed (hemoglobin, urea, glucose, vitamin D, cholesterol, bilirubin, ALT, AST, total protein). The functional status (basic and instrumental functional activity, muscle strength), cognitive functions, dementia and depression, as well as the social status of patients (presence of a disability group, living conditions, presence of an attached serving social worker) were assessed. Results and discussion: 54 patients (12%) belonged to the group of bedridden patients, their average age was significantly lower than the age of patients with limited mobility (77,7±9,5 versus 81,5±8,8 years, respectively, p = 0.008). There were no differences in the incidence pattern between the groups of patients with limited mobility and bedridden, with the exception of one nosology - anemia, which was twice as common in the group of bedridden patients than in the group of patients with limited mobility (60 versus 30%, respectively, p = 0.000). Geriatric syndromes were common among all patients of the patronage service, with which the main differences between the bedridden and low-mobility patients groups were associated (decreased vision, incontinence, insufficient nutrition, decreased functional activity, cognitive impairment, depression, insomnia, chronic pain syndrome).
