PATHOGENETIC SIGNIFICANCE OF VITAMIN D DEFICIENCY AND INSUFFICIENCY IN MAINTAINING SYSTEMIC CHRONIC INFLAMMATION IN ELDERLY PATIENTS WITH LARYNGEAL CANCER IN COMBINATION WITH COPD

Summary:
Introduction. Every year, the number of elderly patients turning to doctors with comorbid pathology increases. The development and progression of oncological diseases and COPD is associated with systemic inflammation. To develop treatment and prevention algorithms, it is necessary to know the mechanisms of maintaining systemic chronic inflammation in elderly patients. Aim: to study the level of vitamin D and its effect on markers of systemic inflammation in elderly patients with laryngeal cancer in combination with COPD and in the isolated course of diseases. Materials and methods. 116 patients aged 60 to 74 years were observed. The main group consisted of 63 patients with COPD II st. and laryngeal cancer 1-2 st. The first comparison group consisted of 26 patients with laryngeal cancer without COPD, the second - 27 patients with exacerbation of COPD II without laryngeal cancer. Levels of vitamin D, IL-4, IL-8, TNF-α, CRP and indicators of the blood coagulation system were determined in all patients. A correlation analysis was performed between the amount of vitamin D and markers of systemic inflammation. Results. The incidence of vitamin D deficiency/insufficiency in elderly patients with laryngeal cancer and COPD is higher than in patients in the first and second comparison groups: 90.5%; 88.5% and 81.5%, respectively. The average vitamin D level in comorbid pathology was 17.1 ± 1.4 ng/ml, in COPD patients - 23.4 ± 1.6 ng/ml, laryngeal cancer - 20.1 ± 0.5 ng/ml. The cytokine status imbalance was significantly higher in elderly people with laryngeal cancer and COPD compared with isolated pathology. Reliable correlations were obtained between vitamin D levels and cytokine status, CRP. The patients of the main group had a high incidence of laboratory hypercoagulation syndrome - 76.2% of cases compared to the comparison groups. PTI was significantly negatively correlated with the study in vitamin D levels. Conclusions. Vitamin D deficiency/insufficiency has pathogenetic significance in maintaining chronic inflammation in patients with comorbid pathology (laryngeal cancer and COPD). The materials of the article will be useful to practical healthcare professionals in the preparation of preventive and rehabilitation programs for elderly patients.
