PROGNOSTIC POTENTIAL OF BRAIN NATRIURETIC PEPTIDE AND CARDIAC TROPONIN I IN OLD PATIENTS DIAGNOSED WITH COVID-19

Summary:
Introduction: During epidemics, ways are required to quickly and accurately diagnose and assess the condition of patients, correctly chosen treatment tactics and predict the course of the disease, which is especially relevant among people over 60 years of age. The most promising area for study is analytes on the one hand related to the state of the cardiovascular system, on the other - an inflammatory response. The purpose: to analyze the peculiarities of the level in the serum of the cerebral natriuretic peptide and cardiac troponin I in a section of different ages in COVID-19. Materials and methods: Total number of patients - 286 people, average age 61.1 ± 17.3 years, study endpoints: adverse outcome - fatal and stay in the intensive care unit for more than 5 days, favorable - discharge from the hospital without a stay in the intensive care unit. Results: Blood levels of both BNP and cardiac troponin I by age showed a single-digit upward trend. A comparative assessment of the level of analytes depending on the outcome of the disease showed significant differences: the concentration of absolutely all the considered inflammatory factors was an order of magnitude higher in patients with an unfavorable outcome of COVID-19. Discussion: It is worth noting that the following age-associated pattern takes place: the concentration of both analytes increases statically with age. These biologically active substances are an indicator of the morphofunctional state of the cardiovascular system. A statistically significant increase in both BNP and troponin I levels in the blood in COVID-19 is clearly age-associated. Conclusion: The findings support the high prognostic value of blood levels of brain natriuretic peptide and troponin I in both middle-aged and older-aged individuals. Blood concentrations of both analytes showed an upward trend in both the age aspect and the adverse outcome of COVID-19.
