ABOUT PROBLEMS AND THE PROSPECTS OF OBLIGATORY MEDICAL INSURANCE IN RUSSIA

Summary:
COVID-19 which arose in addition to a pandemic in February, 2022 the geopolitical and geo-economics conflict with epicenter in Ukraine which is followed by a global economic crisis and mass movements of people will have consequences for public health care of the countries of Western and Eastern Europe, including Russia. It causes relevance of the researches aimed at finding the most effective rules and funding mechanisms for health care. Research objective: the analysis of modern problems and consideration of prospects of development of a system of obligatory medical insurance in Russia Technique and sources of information. In work, data and metadata on the countries of the Organization for Economic Cooperation and Development are used (https://stats.oecd.org/). Data on expenses on medical care are taken from the summary report across Russia on form No. 62 "Data on implementation of the Program of the state guarantees of rendering to citizens of the Russian Federation of free medical care". For the analysis of personnel resources of health care, the data which are contained in forms of the state statistical observation No. 30 "Data on the medical organization" were studied. Results. For all the time of the existence the compulsory health insurance system couldn't correct structural shortcomings of volumes of the provided medical care under the terms of its representation. The medical care provided in conditions of the round-the-clock hospital continues to consume nearly a half of all resources directed to delivery of health care that is 2-3 times more, than in EU countries. The number of doctors to the Russian health care system grows in the basic at the expense of the experts working in the round-the-clock hospital. In the period of COVID-19 pandemic in Russia the regulations which are actually canceling action of Rules of obligatory medical insurance were adopted. Conclusions. The OMC system implemented in Russia during an era of financial capitalism so far sputtered out. Taking into account the remaining extremely low share of the national GDP directed to health care in conditions, the flowing geopolitical and geo-economics crises, the mechanism and sources of financing of health care need transformation. It is possible to refer full cancellation to radical transformations obligatory medical insurance in the Russian Federation. In the state health care system the structures capable to fast mobilization intended for neutralization of biological and military threats of all types have to be created. It is possible to carry an exception of medical insurance companies of the system of participants of obligatory medical insurance, introduction in territorial subjects of the Russian Federation of a system of "the uniform payer" of medical care represented by territorial social insurance funds to the adjusting transformations – branches of the compulsory health insurance Federal fund.
