THE ROLE AND POSSIBILITIES OF MEDICAL CARE PROCEDURES IN THE MANAGEMENT OF A MEDICAL ORGANIZATION

Summary:
Introduction. Since 2012, medical care in the Russian Federation has been provided in accordance with the procedures for its provision (hereinafter referred to as the Procedures), compliance with which for medical organizations is not only a licensing requirement, but also a determining circumstance in the formation of criteria for assessing the quality of medical care, which allows using the Procedures as a necessary tool in the work of the head of a medical organization. The appearance of such regulatory legal documents is assessed by researchers as a positive and important phenomenon, since the Procedures determine an integrated approach to the organization of medical care and take into account the requirements for the provision of medical care. Currently, the list of procedures governing the provision of medical care is quite diverse. However, the problem of the heterogeneity of the field of regulation and the scope of application of the Procedures makes it difficult to use them in the management of a medical organization.
The purpose: to analyze and systematize the procedures for providing medical care for the subsequent development of common management tools for a medical organization.
Materials and methods. The study used methods of comparative and content analysis. Using the content analysis method, the content of text arrays of 93 orders of the Ministry of Health and Social Development of Russia and the Ministry of Health of Russia approving the procedures was studied. With the help of a comparative analysis, 64 procedures for providing medical care were studied, for which an Order analysis Map was developed.
Results. Content analysis of the content of text arrays of orders revealed that Orders are the main regulator of the process of providing medical care, they can determine the provision of assistance to the entire population, or only to adults or children. The procedures are difficult to classify due to the lack of unified approaches to development, which, accordingly, causes difficulties for the heads of medical organizations making managerial decisions when organizing medical care, especially in multidisciplinary healthcare organizations. The procedures for providing medical care can be divided into four groups.
Conclusions. The lack of unified approaches to the development of procedures makes it difficult to use them to manage the resources of a medical organization. The unresolved problem of applying the procedures for providing medical care in healthcare remains the discrepancy between the recommended staffing standards specified in the Procedures and other regulatory documents, which makes it difficult to use them in management activities. In addition, the use of Procedures reduces the possibility of differentiating the resource provision of medical organizations depending on the conditions prevailing in the subject of the Russian Federation (demographic situation, morbidity rate, features of the population).
