INFLUENCE OF THE DYNAMICS OF THE PUMPING FUNCTION OF THE HEART IN ELDERLY PATIENTS SUFFERING IHD AFTER AORTO-CORONARY BYPASS GRAPTION ON CHANGES IN COGNITIVE STATUS IN THE PERIO-OPERATIVE PERIOD

Summary:
Postoperative cognitive dysfunction (POCD) is one of the most common complications of coronary artery bypass grafting (CABG) with cardiopulmonary bypass (CPB). The aim of the work is to study the influence of the dynamics of the pumping function of the heart on the dynamics of the cognitive abilities of elderly patients with coronary artery disease using the “Clock Drawing Test” (CDT) in the perioperative period of CABG with CPB.
Materials and methods. 60 patients with ischemic heart disease, stenosing coronary atherosclerosis, functional class III-IV angina pectoris, who underwent coronary bypass grafting under cardiopulmonary bypass and pharmaco-cold blood cardioplegia, who were being treated in the cardiosurgical department of the Belgorod Regional St. Ioasaph Clinical Hospital (Belgorod), from 2019 to 2021
Results and discussion. As a result of a survey of 60 patients (30 middle-aged, 30 elderly) in the perioperative period of coronary bypass grafting, a direct correlation was proved between the postoperative results of CDT and the perioperative dynamics of the left ventricular ejection fraction (LVEF) (R=0.385) (p=0.035 ) in elderly patients, which was not observed in middle-aged patients. A direct correlation was also found between the dynamics of the results of CDT and the perioperative dynamics of LVEF (R=0.516) (p=0.003). From which it can be concluded that the dynamics of the pumping function of the heart can directly affect the dynamics of the cognitive status in elderly patients with coronary artery disease in the perioperative period of CABG with CPB.
Conclusions. This study established the age-related features of the formation of cognitive deficits in elderly patients with coronary artery disease who underwent CABG with CPB.
