CLINICAL AND ECONOMIC JUSTIFICATION FOR THE INCLUSION OF NEW ENTERAL MIXTURES IN THE LIST OF SPECIALIZED PRODUCTS THERAPEUTIC NUTRITION FOR DISABLED CHILDREN DIAGNOSED WITH CYSTIC FIBROSIS

Summary:
Cystic fibrosis (MV) is one of the most frequent monogenically inherited diseases among orphan diseases (the frequency of newborns in Russia is 1:9000). MV is a chronic progressive multisystem disease affecting the respiratory tract, gastrointestinal tract, liver, pancreas, salivary, sweat glands, and reproductive system. Malnutrition is one of the main problems of patients with MB, which contributes to a deterioration in the quality of life and a faster progression of the disease. A direct relationship between good nutritional status, improved lung function and survival has been proven. Therefore, patients with MB from the moment of diagnosis need an increased intake of protein by 50% - 100% and energy compared to their peers. Protein sources are well-known products: meat, poultry, fish, seafood, milk, dairy products, cottage cheese, cheeses, eggs. As an additional source of protein, it is recommended to use therapeutic mixtures for enteral and supplementary nutrition with a high protein content. The goal is a clinical and economic justification for the inclusion of new therapeutic nutrition products in the list of specialized therapeutic nutrition products for disabled children in order to save the program budget. Material and methods: the analysis of 3722 patients with cystic fibrosis (hereinafter referred to as MV) who are in the register in 2020 of the Academician N.P. Bochkov Medical and Genetic Research Center, which is published in open access on the organization's website, depending on gender and age, was carried out. The analysis of clinical recommendations on protein replenishment in children diagnosed with cystic fibrosis was carried out. The analysis of the impact on the budget of the list of specialized medical nutrition products for disabled children with the inclusion of new enteral mixtures was carried out. Results. The analysis of the ratio of protein concentration in medical nutrition products, which are included in the list of specialized medical nutrition products for disabled children, is presented. A model of a patient with MB has been formed, who is shown medical nutrition products, taking into account the frequency of service provision and frequency of use according to the recommendations on cost formation for clinical and statistical groups of the state guarantee program. The analysis of the impact on the budget of the program when new therapeutic nutrition products are included is carried out. Conclusions. The most concentrated protein is the product Frezubin Protein dry mix (Fresenius- Kabi, Germany), which contains 6.9 g of protein per 100 ml, this product dissolves in any liquid or semi-liquid food consumed by the patient and does not require additional water load. All other medical nutrition products, which are included in the list of specialized medical nutrition products for disabled children, not only have a lower protein concentration in 100 ml of solution, but also require a large amount of liquid to assimilate this protein. For a 10-year-old girl or boy, it is necessary to drink from 2 to 3 liters of the prepared mixture of Nutrizon, Peptamen Junior or any of the domestic NUTRIEN products daily, which entails the question of the commitment of patients and their parents to such protein replenishment. At the moment, the list of specialized therapeutic nutrition products for disabled children does not include the highest protein product, Frezubin Protein, which has advantages in adherence to consumption and the absence of additional water load in patients with MV in any age group.
