PROBLEMS OF ACCOUNTING FOR ADVERSE EVENTS IN MEDICAL ACTIVITIES (BASED ON THE RESULTS OF QUESTIONNAIRE SURVEY OF HEADS OF MEDICAL ORGANIZATIONS)

Summary:
Introduction (relevance): in accordance with the order of the Ministry of Health of the Russian Federation from 31.07.2020 № 785n "On Approval of Requirements for the organization and conduct of internal control of quality and safety of medical activities" each medical organization should take into account undesirable events (including potential ones) that occurred during the implementation of medical activities, analyze and take measures to prevent them. As practice shows, this requirement of the industry order is not fulfilled in all medical organizations: some of them have a systematic approach to risk management, while others have a rather formal approach that does not bring results. The problem lies in the absence of a unified classification of undesirable events and a single mechanism for their accounting, which forces managers to independently determine the forms and methods of work to ensure this activity. Objective: to study the opinion of managers of medical organizations of the public health sector on the implementation of the requirements of the Order of the Ministry of Health of Russia from 31.07.2020 № 785n on accounting for adverse events arising in the course of medical activities. Materials and methods: in order to study the opinion of heads of medical organizations, a sociological study was conducted using the survey method and the method of analysis of the obtained information. To conduct the survey, a questionnaire containing twenty questions on the topic of the study was prepared. The obtained information was analyzed by calculating and comparing extensive and intensive indicators. Results: the survey confirmed the existence of certain difficulties in the implementation of regulatory requirements for the consideration of adverse events by medical organizations and the development of measures for their prevention. The peculiarities of the region, consisting in the presence of medical organizations remote from decision-making centers and lack of qualified personnel, aggravate these difficulties. The majority of respondents understand the importance of accounting for adverse events and express high readiness to openly discuss them on official platforms, and to improve the effectiveness of organizational measures they consider it necessary to have a unified classification of adverse events in the region and a unified approach to their identification, registration, development of preventive measures and motivation of employees to continuously improve processes related to professional activities. Conclusions: The study revealed a number of key aspects that require further attention and policy adjustments in the area of adverse event reporting. It clearly points to the need for further work in this area, from staff training to the development of regionally consistent databases and mechanisms for implementing sectoral requirements, taking into account local and facility specificities for more effective implementation of innovations. It may be necessary to develop region-specific recommendations or training programs to help specialists effectively cope with the tasks they face, as well as to develop and replicate best practices to motivate staff to actively participate in the process of improving the quality of medical services.
